Description
To whom it may concern; The Department of Veterans Affairs intends to enter into a purchase agreement with VENDOR: Champion Medical Tech. 765 Ela Rd Suite 200 Lake Zurich, IL 60047-6305 Champion Medical Tech a small business is the sole distributor of the UDIT TRACKER SYSTEM. The North American Classification System (NAICS) code is 339112. Any interested parties may identify their capabilities and interest in responding to the requirement and furnish detailed data concerning their capabilities to provide the product. This notice is not a request for competitive quotes; however, any firm that believes it can meet the requirements must provide written notification to the Contracting Officer by 12/02/2014 at 12 Noon. Information should be sent to Sean Powers Contract Specialist at sean.powers2@va.gov. Information received will be considered solely for the purpose of determining whether this Item is indeed a sole source or if we can conduct a competitive requirement. If no responses are received by the response date and time, The Department of Veterans Affairs will proceed with the intended agreement. All questions concerning this requirement must be in writing and submitted via email to sean.powers2@va.gov. Statement of Work System: Biological and Non-Biological Implant Tracking System Acquisition will include the purchase of a Biological and Non-Biological Implant Tracking System. The system must be robust, and meet our privacy, record management, and security requirements. The vendor must provide the VA security/privacy compliance approval document if the data is physically housed offsite of the VA Connecticut Healthcare System- West Haven campus. Vendor is to provide all hardware, software, and installation for a Biological and Non-Biological Implant Tracking System. The installation should include initial system setup and include entering all current inventories into the system. The vendor will test the system in conjunction with our existing equipment, provide technical training, user manuals, service manuals, training to the staff, and phone support. The VA will procure one freezer capable of achieving temperature up to - 80 degrees Celsius. The freezer shall not exceed 14.9 cu ft. This freezer should also have a backup mechanism in case power is lost to the unit, such as CO2. The system must have an ambient temperature storage cabinet option so the facility is able to grow this tracking system in the future. The software must provide single user authentication for tracking purposes such as Keycard access. It also must have tiered access for control of data security. The system must be compatible with our existing scanners (tethered and wireless) and Barcode Printers which allows for time savings and the elimination of manual data entry errors. The system must have an automated Recall management system that is able to match FDA and manufacturer recalls with the hospital inventory. A manual method of recall matching is not acceptable. The System must have an area to store tissue prep instructions to eliminate a paper binder. The system must have the ability to track all implantable devices across hospital departments. This includes any item with a lot/serial/UDI number. The system must have the ability to provide regulatory status of suppliers for AATB and FDA that is updated and maintained on a reoccurring basis. The system must have the ability to track the temperature of the tissues. The system must have the ability to return tissue implant records via email to the supplier. Scheduling and Hours of Work: Work shall be scheduled for proper execution to completion of this contract. All work in areas involved shall be scheduled with the VA Supervisor. The contractor shall perform this work during normal business hours.
