TELEVISION CHANNEL RECEPTION SERVICES - VA MEDICAL CENTER, BATH, NY 14810 PERIOD OF SERVICE: DECEMBER 1, 2014 THROUGH SEPTEMBER 30, 2015 Bath VA Medical Center Television Reception Service Statement of Work December 1, 2014 to September 30, 2015 Summary: This contract request is for the provision of television reception and programming for the Bath VA Medical Center, Bath, NY. Reception service is currently provided to approximately 225 outlets at various locations throughout the VAMC Campus. Locations: VA Medical Center 76 Veterans Avenue Bath, NY 14810 Scope: Approximately 225 televisions are to be provided programing through this requested service. This number is subject to variation depending on the needs of the organization as areas may change due to minor renovations or changing needs of the organization. Contractor will provide services to approximately 225 locations/connections with a 15% quantity variation (minimum of 192 and max of 258). Pricing will be provided on a per outlet basis for proper cost accounting. Requirements: Contractor will provide basic television reception for televisions dedicated for Veteran use in common areas and patient/resident rooms. Programing must include basic video programing including local network affiliates, ABC, CBS, FOX, NBC, etc. Channels shall to be broadcast will be 2-79. The signal will be provided in the North Service Room of Building 76 and will be connected to a VA owned cable infrastructure that has VA channels injected above channel 79. The signal must be analog to support the existing system. Award will be based on lowest price technically acceptable. The signal provided will be able to be broadcast in a majority of our buildings on the facility. Period of service will start December 1, 2014 and end on September 30, 2015. The 10-month period will be invoiced in 10 equally priced per month invoices that are to be paid in arrears. Contractor is responsible for any installation costs associated with this agreement, including any equipment and labor needed to provide service. Installation costs must be noted as a separate line item associated with the base award. All equipment costs shall be included in the service price, exclusive of installation costs.
