- Current Wage Determination Past Performance Questionnaire Request for Quotes (RFQ) Performance Work Statement (PWS) Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (ii) The solicitation number F3CTSW4294AW01 is being issued as a request for quotation. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-77. (iv) This acquisition will be made under 100 percent Full and Open Competition. The North American Industry Classification System (NAICS) code is 541690. (v) CLIN 0001 - Comprehensive Safety Assessment of the roads and driving conditions for the missile fields, follow up by a report. IAW attached Performance Work Statement (PWS) (Attachment 1) (Qty 1 Each). CLIN 0002 - 4 Hour Defensive Driving Course for 25 to 50 people per Session IAW PWS (Qty 36 Sessions). CLIN 0003 - 4 Hour Winter Driving Course for 25 to 50 people per Session IAW PWS (Qty 36 Sessions). Please provide quote on the attached Request for Quotation (Attachment 2). (vi) The 91 MW requires a comprehensive safety assessment to identify, quantify and prioritize the most common and most likely risks and hazards to its vehicles and driver. A report and briefing will be required as well as a 4 Hour Defensive Driving Course and 4 Hour Winter Driving Course IAW the criteria of the Performance Work Statement. (vii) Place of delivery is Minot AFB, ND 58705 and period of performance for CLIN 0001 will be date of award through 60 days after award. CLIN 0002 will be date of award through 30 Sep 2015 and CLIN 0003 will be date of award through 30 April 2015. (viii) The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The government will award a firm-fixed price contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Award shall be based on best value to the government. The evaluation procedure to be used for this procurement is price and other related factors. The other factors are as follows: the Offeror must have successfully completed two contracts with a minimum value of $25,000 annually and be actively engaged as a contractor performing services as defined by the NAICS Code 541690 within the last five years (See Attachment 3). The Offeror must provide a timeline and plan on how the requirements stipulated in the PWS are going to be met. (x) The government advises Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Offerors are also reminded to complete Offeror Representations and Certifications on-line at www.bpn.gov prior to submitting an offer. (xi) Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xii) Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.204-10, 52.209-6, 52.222-3, 21, 26, 52.222-36, 52.223-18, 52.225-13, 52.232-33, 52.222-41, and 52.222-42. Current Wage Determination is attached (Attachment 4). (xv) All offerors must submit a quote by 4:30 pm Central Standard Time on Friday, 5 December 2014. Quotes received after that time may not be considered.
