Description
This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size classification relative to NACIS 811219. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. The G.V. Montgomery Veteran Affairs Medical Center in Jackson, MS is seeking responsible sources for a potential contract to provide preventive maintenance, emergency repairs, and chamber cleaning for four (4) STERIS sterilizers. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, and business size. Responses to this notice shall be e-mailed to Timothy.Pighee@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 5:00 p.m. EST November 24, 2014. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. STATEMENT OF WORK STERIS Sterilizers 1.SCOPE: The G.V. (Sonny) Montgomery Veterans Affairs Medical Center (GVSMVAMC) requires comprehensive service contract on four (4) STERIS Sterilizers. 2.Definitions: COR: Contracting Officer's Representative GVSMVAMC: G.V. (Sonny) Montgomery Veterans Affairs Medical Center VHA: Veterans Health Administration VA: Department of Veterans Affairs VA Mission: "To fulfill President Lincoln's promise "To care for him who shall have borne the battle, and for his widow, and his orphan" by serving and honoring the men and women who are America's veterans." 3.BACKGROUND: The GVSMVAMC has three (3) STERIS Sterilizers located in SPS and one (1) located in Surgery. Due to the complexity of these sterilizers a service contract for preventive maintenance/emergency repair and chamber cleaning is required. 4.SUMMARY OF REQUIREMENTS: Contractor shall provide preventive maintenance (PM) and support to: "STERIS 48" Platform Prevacuum Sterilizer, Single hinge, Right-Hand Ressecessed, 208/240V "AMSCO 400 12x12x26 Prevacuum Sterilizer 5.TASKS Requirements: Service Calls: On site response will be within 24 hours. Performance: All work is to be performed by competent personnel, experienced, and certified to work on the STERIS Sterilizers. Preventive maintenance, to include all parts and labor in accordance with the manufacturer's Preventive Maintenance Check List ("PMCL") will be accomplished two (2) times per year. One (1) chamber cleaning per year will be accomplished on each unit. Unlimited emergency repair visits will also include parts and labor. These calls will be responded to within 24 hours Equipment: Government property cannot be removed from the station without a signed property pass. This pass can be obtained after removal is authorized (by the COR) from Logistics Service. The contractor shall be responsible for loss or damage of equipment while in the contractor's charge. Service Call: There will be unlimited service visits during the period, Monday through Friday from 8:00 AM to 4:30 PM, excluding holidays. Help Desk Support: Technical and clinical telephone and email support will be provided twenty-four hours per day, seven days per week. Holidays that fall on Sunday are observed on the following Monday.
