Description
DEPARTMENT OF VETERANS AFFAIRS FAR Part 6 Justification and Approval For Other Than Full and Open Competition 1. Contracting Activity: Department of Veterans Affairs, VISN 23, 4101 Woolworth Ave. Omaha, NE 68105 2.Nature and/or Description of the Action Being Processed: The Omaha VA is seeking approval to award a sole source contract to Urosource Mobile Medical Solutions Inc., for RF, MicroWave and Cryoablation procedures, to include the RF, MicroWave and Cryo unit and technical staff for each case for one six (6) month plus one six (6) month extension. Price fluctuation should be built in to account for any increase in procedure cost on the open market. 3.Description of Supplies/Services Required to Meet the Agencv's Needs: UroSource will work with the Omaha VA to perform the RF, Microwave and Cryoablation procedures according to our schedule. UroSource will supply equipment and technical staff for each procedure. 4.Statutory Authority Permitting Other than Full and Open Competition: ( ) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements per FAR 6.302-1; ( x ) (2) Unusual and Compelling Urgency per FAR 6.302-2; ( ) (3) Industrial Mobilization,Engineering, Developmental or Research Capability or Expert Services per FAR 6.302-3; ( ) (4) International Agreement per FAR 6.302-4 ( ) (S) Authorized or Required by Statute FAR 6.302-5; ( ) (6) National Security per FAR 6.302-6; ( ) (7) Public Interest per FAR 6.302-7; 5.Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicabilitv of authority); The current incumbent vendor is currently in place and has a satisfactory performance record. This contract is a stop-gap measure to provide mission essential and critical patient services to the veterans at the VA Hospital. 6.Description otEfforts Made to ensure that offers are soli cited from as many potential sources as deemed practicable: The Government intends to fully compete these services on the follow-up successor contract. 7.Determination by the Contracti ng Officer that the Anticipated Cost to the Government will be Fair and Reasonable: The Contracting Officer will determine the price to be fair and reasonable. 8.Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: There is inadequate time for market research to be performed. These are mission essential critical patient services for the VA. 9.Any Other Facts Supporti ng the Use of Other than Full and Open Competition: N/A 10.A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: The Government intends to compete the successor contract. 11.Requirements Certification: Icertify that the requirement outlined in this justificati on is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
