- Justification and Approval Attachment 6 - Statement of Work Attachment 5 - Quarterly Sales Activity Report Attachment 4 - Pricing Matrix Attachment 3 - Past Performance Questionnaire Attachment 2 - Master Item List Attachment 1 - RFP Checklist Solicitation The Department of Homeland Security (DHS), U.S. Customs and Border Protection (CBP) is issuing a Request for Proposal (RFP) for medical supplies. CBP and other DHS Components require vendor(s) to supply medical supplies used by personnel to train for and perform medical treatments, tests, and operations. Supplies are those used in a pre-hospital, emergency medical services (EMS) context, and oftentimes in an austere, remote environment. This is a strategic sourcing initiative that will result in a multiple award DHS-wide Blanket Purchase Agreement (BPA) available to all Components for use. Upon award, this contract vehicle will be mandatory for use per DHS Directive 060-01. Other DHS Components that use medical supplies include Federal Protective Service (FPS), Federal Emergency Management Agency (FEMA), Federal Law Enforcement Training Center (FLETC), Immigration and Customs Enforcement (ICE), U.S. Citizenship and Immigration Services (USCIS), U.S. Coast Guard (USCG), and U.S. Secret Service (USSS). CBP is issuing this solicitation under NAICS code 423450 and in accordance with FAR Part 12. The scope of this BPA is to provide organizational components within DHS various types and brands of medical supplies for delivery to various CONUS and OCONUS locations. Contractors shall provide access to medical supplies as made available through their government or commercial catalogs. All commercial medical supply offerings available through the vendor's commercial or government catalog may be purchased through this BPA. All medical supplies within the scope of this BPA are identified as and determined to be commercial items readily available in the commercial marketplace. Contractors shall furnish medical supplies used by DHS personnel to train for and perform medical treatments, tests, and operations. Offerors have the ability to propose on one or more categories as determined by the offeror. The categories are as follows: General Medical Supplies Automatic External Defibrillators (AED)/Heart Monitors Kits (Small Businesses Only) Awards will be made on a lowest price technically acceptable basis to the offerors who propose a solution that is in the best interest of the Government.
