- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (i) The solicitation number and a statement that the solicitation is issued as a request for quotation (RFQ) VA259-15-0048 (ii) A notice regarding any set-aside and the associated NAICS code 334510 and 750 employees small business size standard. (iii) A list of contract line item number(s) and items, quantities and units of measure are listed in the RFQ (iv) Description of requirements for the items to be acquired. (v) Items are to be delivered to VA HEALTHCARE SYSTEM 500 FOOTHILL RD SALT LAKE CITY UT 84148 (vi) Provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (vii) Best Value to the government along with price shall be evaluation factors (viii) Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (ix) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. (x) Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. (xi) Quotes are due into Contracting Office ( address on RFQ) 12 Nov 2014 10:00 a.m. MST. Quotes may be submitted via USPS or email.
