Description
THERE IS NO SOLICITATION AVAILABLE AT THIS TIME. REQUEST FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. This Sources Sought Synopsis is in support of Market Research being conducted by the Missouri National Guard to identify potential Medical Services Contractors, and if this effort can be competitive or a Small Business Set-Aside. The proposed North American Industry Classification Systems (NAICS) Code is 561320 which maintains a corresponding Size Standard of $27.5M in annual revenues. The Government will use this information to determine the best acquisition strategy for this procurement. The Government is interested in all businesses to include 8(a), small business and small disadvantaged business, Service-Disabled Veteran-Owned, Hubzone, and Women-Owned small business concerns. The government requests that interested parties respond to this notice if applicable and identify your small business status to the identified NAICS code. Additionally, please provide any anticipated teaming arrangements, along with a description of similar services offered to the Government and to commercial customers for the past three years. Any responses involving teaming agreements should delineate between the work that will be accomplished by the prime and the work accomplished by the teaming partners. This Sources Sought is to notify companies that the Missouri National Guard is seeking sources for temporary services to conduct two day medical health assessment for personnel as follows: PERSONNEL QUANTITIES, DATES, AND LOCATIONS 1.January 2015 to December 2015; between 2,500 and 6,000 personnel at multiple locations and various dates/times. KEY MEDICAL/DENTAL TASKS TO BE COMPLETED: 1.Periodic Health Assessments (PHA): The contractor shall conduct PHAs in accordance with the PHA guidance set forth by AR 40-501 (23 04August 2010 2011RAR) 2.Post Deployment Health Re-Assessments (PDHRA): the contractor shall conduct PDHRAs in accordance with AR 40-501 (RAR 003, 08/04/2011) 3.Periodic Health Assessments and Physical Exams: the contractor shall conduct Periodic Health Assessments (PHA) and other physical exams (Over-40, Chapter 2, Flight, etc.) in accordance with AR 40-501 (RAR 003, 08/04/2011) 4.Visual Acuity Testing (Snellan/near vision): The contractor shall conduct visual acuity testing in accordance with AR 40-501 (RAR 003, 08/04/2011) 5.DOEHRS Compatible Audiometer Testing: Portable audiometer testing is not acceptable, audiometer testing must be provided in a mobile facility. The contractor shall conduct audiometry in accordance with AR 40-501 (RAR 003, 08/04/2011) 6.Audiometer must be DOEHRS real time upload compatible. Adherence to all requirements of OSHA, State & Federal Regulations is mandatory. Audiology evaluations and Speech Recognition in Noise Test (SPRINT) testing will be performed IAW all applicable Federal, DOD, MEDCOM, NGB, and State guidelines. SPRINT testing must be available at all events when requested and the results must be available immediately. 7.Immunizations: The contractor shall conduct immunizations in accordance with AR 40-501 (RAR 003, 08/04/2011) 8.Phlebotomy/Lab: the contractor shall conduct all laboratory / phlebotomy services such as HIV, Titer, G6PD blood draws, pregnancy testing, and lipid profiles, as well as flight physical or special forces unique blood draws IAW AR 40-501 (RAR 003, 08/04/2011) 9.DNA Collection: The contractor shall conduct DNA collection in accordance with the AR 40-501 (RAR 003, 08/04/2011) KEY DENTAL TASKS TO BE COMPLETED 1.Digital Bitewings, Digital Panoramic Radiographs & Dental Exams: The contractor shall perform required dental examinations to include digital bitewings and digital panoramic radiographs if clinically indicated in support of the annual dental screenings IAW AR 40-501 (RAR 003, 08/04/2011) 2.Diagnostic Services Performed a.DOl20-Periodic Oral Evaluation b.DOl50-Comprehensive Oral Evaluation c.D0330-Panorarnic Film d.D0272-Bitewings-Two Films e.D0274-Bitewings-Four Films f.D02201D0230- Periapical (limited) 3.Exam. A periodic oral evaluation shall assess the current state of oral health and risk for future dental disease, as well as assessing general health factors that relate to the treatment of the patient. They must know the requirements of Dental Readiness for the state of Missouri and be proficient with DENCLASS and MEDPROS. HIPAA certified Privacy Act OSHA & ADA compliant Special Qualifications: The contractor is responsible for ensuring that all staff members are certified dental professionals. All dental and medical assistants and staff shall be trained and professional with good morals and ethics. Technical Orders and Qualification Requirements are not available.
