- SUBJECT: Request for information (RFI), Synopsis for Laser Safety Glasses and Goggles. The Department of Veterans Affairs, in conjunction with the Veterans Health Administration (VHA) is seeking a better understanding of the market capability for the proposed commodities above. The purpose of this Request for information (RFI) is to obtain information on the capability of vendors to provide Laser Safety Glasses and Goggles to protect the eyes of doctors and patient during surgical or dental procedures. Protective eyewear is extremely important for protecting the eyes from different wavelengths of laser lights during various medical procedures. The unprotected human eye is particularly sensitive to laser radiation and can be permanently damaged from direct or reflected beams. This RFI allows potential contractor to submit a non-binding statement of interest and documentation demonstrating their capabilities, and suggestions that will allow the Government to refine its potential course of action. The RFI effort is intended to assess available sources of supplies, suppliers' capabilities, and solicit feedback for acquisition planning purposes. The strategic Acquisition center (SAC) is requesting response from qualified suppliers and/or businesses concerns. The NAICS code for this effort is 621493. BACKGROUND: VHA is comprised of 152 medical centers that perform Surgical and Dental procedures on patients which sometimes involved the use of lasers. The medical facilities are located throughout the United States. Laser Safety Glasses and Goggles are typically needed for eyewear protection. This eyewear must protect doctors and patients from various wavelengths of light. The Government's and the VHA's objective is to achieve standardization of uniform, quality product, at the lowest cost. This standardization will ensure continuity of care from one VA hospital to another across the entire country. SPECIFIC MANDATORY TASK AND ASSOCIATED DELIVERABLES. Task 1 - Contractor will be required to deliver FDA Approved Laser Safety Glasses or Goggles to the specified address by the time and date specified. Task 2 - Contractor will be required to deliver the FDA approved Laser Safety Glasses or Goggles to the specified address Free on Board (FOB) to any destination within the United States. Task 3 - Contractor will be required to meet the government's quality assurance standard as called out in standard commercial language and the contractor shall give their assurance that the FDA approve product conforms to the contract requirements. Task 4 - Contractor will be required to provide standard commercial warranty of merchantability and fitness for particular purposes and remedies. Task 5 -Contractor will possess Electronic Data Interchange (EDI) capability for orders receipt, order confirmation, functional acknowledgement, and invoice. INSTRUCTIONS: Submit a brief description, four (4) pages or less (including cover letter) that demonstrates your company's ability to provide laser safety glasses and goggles that can protect the doctors and patients (users) from the varying wavelengths of laser lights during prescribe medical or dental procedures. You should also demonstrate your company's ability to provide this product anywhere in the United States. Demonstrate your past experience in performing these services for the past two years to include your contract number and your socioeconomic status. Responses should include business size, business type, socio-economic status, and capability. Please submit your responses to the Contracting Specialist, Maurice Spencer at Maurice.spencer@va.gov, Responses are due by 12:00 noon EST November 17, 2014. No solicitation documents exist at this time; the Department of Veterans Affairs is neither seeking proposals nor accepting unsolicited proposals, and responses to this RFI or Sources Sought cannot be accepted as offers. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Any information the vendor considers proprietary should be clearly marked as such. The U.S. Government will not pay for any information or administrative costs incurred in response to this RFI or Sources Sought.
