- ****Request for information only**** FBO ID# VA243-15-N-0086 Response date (Deadline): NOVEMBER 4, 2014 Sources Sought: SDVOSB Sources to Furnish Eyewear to VISN3 This request for information (RFI) is posted for market research purposes ONLY. It is not a solicitation. Statement of need: The Department of Veterans Affairs New York/New Jersey Healthcare System (VISN 3) has an ongoing program to furnish eyeglasses to those Veterans that require them and are eligible for assistance Consistent with Public Law 109-461, VISN3 is seeking Service-Disabled Veteran Owned Small Business sources to provide these eyeglasses and related services, The chart below shows the approximate breakdown of prescription eyeglasses by type for FY13. Add-ons, such as tints, ultra violet, and anti-reflective coatings were included in the prescribed eyeglasses breakdown below. Product or service description: "The Contractor shall fabricate all eyeglasses in strict conformance to the eyeglass prescription provided by the VA Clinical Team, Prosthetic Representative, and/or designee. All items provided by the contractor shall be new and first quality from original manufacturer. "All frames shall be fabricated in accordance with the current edition of the American National Standard Requirements for Dress Ophthalmic Frames ANSI Z80.5 latest edition.. All items provided by the contractor shall be new. The Contractor shall provide completely assembled eyeglass frames placed in standard alignment and include mounting of lenses. "The following facilities will provide space for an on-site fitting area as available: Manhattan, Brooklyn, St Albans, Castle Point, Montrose, Bronx, and Northport (in New York); East Orange, and Lyons (in New Jersey). "On-site optical services will include, but are not limited to eyeglass selections, measurements, dispensing, fitting and repairing as required. "This contract will not include eye examinations. "The Contractor will provide a licensed American Board Certified Optician (Optical Fitter) with appropriate ophthalmic dispensary training to perform these services at each VA Medical Center in the VISN. The optician shall provide optical services to veterans who are eligible for government provided eyewear, and also be able to verify eyeglass prescription specifications provided by the VAMC providers as indicated here. "The Contractor shall provide a toll-free telephone number for ordering and customer inquiries and shall designate an employee as a VA customer service person to maintain the account and rapidly resolve any identified problems. Local Geographic Area: VISN3 is seeking SDVOSBs whose main office/primary location is within 200 miles from Zip Code 10010 Capability or performance: All contractors and staff provided to work on site must be fully accredited, certified and licensed in the states of New York, and New Jersey as applicable and required by law. Type of Procurement: The responses to this Request for Information (RFI) will help determine what, if any, type of set-aside Contract Vehicle: The VA anticipates a single 5-year IDIQ requirements contract or BPA. will be made based on best value to the government to best meet the needs for the veterans in VISN 3.
