Description

COMBINED SYNOPSIS/SOLICITATION This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA119-14-Q-0266 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 05-76, dated 25 Aug 2014. The associated North American Industrial Classification System (NAICS) code for this procurement is 813920. This procurement action is unrestricted. The Department of Veterans Affairs (VA) intends to award a Firm Fixed Price contract, in accordance with FAR Part 16.202, with one-year base and four (4) one-year option periods in support of the VA Office of Pathology and Laboratory Services Office. The purpose of this contract is to fulfill a requirement that all laboratory testing undergo an onsite inspection and be accredited by a nationally-recognized, Center for Medicare and Medicaid Services (CMS)-deemed accreditation body. All work shall be performed within U.S. facilities and U.S. territories. 1.Contract Line Item Numbers (CLINs) are provided in -Schedule B Price Schedule attached hereto. 2.Statement of Work is provided in Attachment 2 - Statement of Work 3.Additional Instructions are found in Attachment 3 - Instructions to Offerors 4.Clauses and Provisions are found in Attachment 4 - Clauses and Provisions 5.The list of Laboratories and their accreditation expiration dates are found in Attachment 5 - List of Laboratories 6.Attachment 6 - Contractor's Rules of Behavior Quotes are due November 7, 2014 at 5:00pm Eastern time.
