Description

Contractor shall provide wheelchair van transportation, when required, to transport wheelchair patients. Patients to be transported will generally be outpatients and will not require medical attention during the trip. Most trips will be with a starting or ending point in the surrounding counties of Outagamie including Waupaca, Waushara, Shawano, Brown, Door County, Kewaunee, Calumet, Winnebago, Fond du Lac, Manitowoc, and Sheboygan counties and start or end at the John H. Bradley Clinic in Appleton, WI Cleveland Outpatient Clinic in Cleveland WI or Milo H. Humphrey clinic in Green Bay WI Contractor shall also provide transportation services for patients within the above stated counties for Non-VA Care appointments in the community. Contractor shall also provide transportation services for patients with appointments at other health care facilities in the Madison, Tomah, Iron Mountain, Milwaukee or Chicago areas. Drivers, attendants, and vehicles must meet the qualifications of standard industry practice and as listed herein. One way trips will not exceed 5 hours from the VA clinics or patient's place of residence.
