Description
Questionnaire DRAFT Package Please find attached questionnaire that you will be able to see Japanese instruction. We are looking for vendors who can do this service. The solicitation has not been released yet. The service will start from 1 Oct 2015. NOTE: ALL SERVICE WILL BE CONDUCTED IN JAPAN. THEREFORE, APPLICABLE JAPANESE LICENCES ARE REQUIRED TO ACCOMPLISH THIS WORKS. The requirements include monthly inspection and preventive maintenance of all elevators, dumbwaiters (collectively "elevator" hereafter) and escalators located in Operation and Maintenance (O&M), Military Family Housing (MFH) and The Department of Defense Dependents Schools (DoDDS) facilities at Yokota Air Base and Tama Hills Recreation Area (Tama Annex), The monthly preventive maintenance includes 1) monthly inspection/maintenance and 2) Repair Service Calls that consist of Routine Calls and Emergency Calls. Routine calls are repair service requests that the Contractor shall respond to the work location within two (2) hours after notification during normal business hours and three (3) hours at times other than normal business hours, whereas emergency calls are repair service requests that the Contractor shall respond to the work location within one (1) hour after notification, and shall restore the failed elevator/escalator to normal operation status at the earliest possible time. Further, Level II work will be used for any repair and restoration of elevator/escalator service using parts or components beyond the scope of preventive maintenance. Maximum amount for level II work is limited to the Yen equivalent of $60,000 per each supplemental agreement in accordance with DRAFT of Performance-Based Work Statement (PWS).
