- (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)The solicitation number is: VA259-15-Q-0031, and it is issued as a request for quotation (RFQ). (iii)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74. (iv)This solicitation is set-aside for Small Business. NAICS: 311423 (v)Contract Line Items: a.Emergency Food Rations (vi)Description of requirements for the items to be acquired (see Statement of Work): STATEMENT OF WORK (SOW) Emergency Food Rations The VA Salt Lake City Healthcare System (VASLCHCS) is in need of emergency food storage in our Food and Nutrition department. This is to comply with our disaster preparedness directive. Emergency food supplies are needed to ensure patient's nutrition requirements are meet during any potential emergency or disaster. The bidding vendor must meet the following requirements: Dehydrated is ok if it reconstitutes easily with just water. No heat required, and includes meat products. Many dehydrated entrées do not have meat so that is why we want freeze-dried. Must feed 600 patients at 1750 kcals daily for 7 days, three meals, 65 grams of protein 25 year shelf life (15 for Gluten Free) Breakfast - 4200 meals - providing an average of 350 kcals per entrée. At least 2 high proteins items requested, including eggs Lunch/Dinner- 8400 meals - providing an average of 350 kcals per entrée- (twice daily), 75% of entrées should contain meat Dessert - 2100 servings - providing an average of 200 kcals per serving (for every other day intake), at least 2 varieties Rice - 2100 servings - providing an average of 200 kcals per serving (for every other day intake) Milk - 8400 servings - providing an average of 100 kcals per serving (twice daily), chocolate and white milk Drinks- 4200 servings - providing an average of 100 kcals per serving (daily), at least 2 flavors Fruit - 8400 servings - providing an average of 55 kcals/per serving (twice daily), at least 4 varieties Vegetables- 8400 servings - providing an average of 55 kcals/per serving (twice daily), at least 4 varieties Gluten free should feed at least 11 people and have similar calories, protein, meat preferences and variety as above. -Breakfast - at least 77 servings -Lunch/Dinner- at least 154 servings -Milk - at least 154 servings -Drinks - at least 77 servings -Rice - at least 40 servings -Fruit - at least 154 servings -Vegetable - at least 154 servings -Dessert - at least 40 servings END OF STATEMENT OF WORK (vii)Date and place of delivery (FOB Destination): 30 Days ARO a.VA Health Care System 500 Foothill Drive Salt Lake City, UT 84148-0001 (viii)The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition.
