Description
SF 1449 - Solicitation, Provisions, and Clauses. Past Performance Questionnaire. The questionnaire is to be returned via e-mail directly to Rhonda Sizemore. See instructions in solicitation. Historical Room Occupancy for Fiscal Year 2014 Historical Room Occupancy for Fiscal Year 2013 A sample sustainable procurements report. This report is to be filed by the Contractor on an annual basis. A blank incident report to be used for reporting incidents occurring on the Academy's premises. A sketch showing the entrances and walkways of the Academy A floor plan showing the MSHA areas to be cleaned in the IUOE Building. A spreadsheet showing the approximate square footage of the various areas of the Academy Collective Bargaining Agreement between Action Facilities Management, Inc. and union: International Technical and Professional Employees Union PWS for Maid, Janitorial, and In-House Laundry Services at the Mine Academy in Beaver, WV This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Solicitation DOL-MSH-15-R-00001 is being issued as a Request for Proposals (RFP), to provide janitorial, maid and in-house laundry services for the National Mine Health & Safety Academy, an division of the U.S. Department of Labor's Mine Safety & Health Administration, located at 1301 Airport Road, Beaver, West Virginia, 25813. This acquisition is set-aside for Woman Owned small businesses. The North American Industry Classification System (NAICS) code is 561720 and the size standard is $18 mil. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-76. NOTICE OF SITE VISIT: A site visit is scheduled at the Mine Safety & Health Administration, 1301 Airport Road, Beaver, West Virginia, 25813, for Thursday, October 30, 2014 beginning at 10:00 a.m. EST. Offerors are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award. Please send an e-mail to Sizemore.Rhonda@dol.gov no later than 1:00 p.m. EST Wednesday, October 29. 2014 providing your company name, address, phone number, DUNS number, and the name(s) of the person(s) that will be attending the site visit. This facility is guarded; therefore, proper identification (Driver's license and/or ID card) is required for admittance. Site visit attendees will report to the Academy's main reception area to wait for the site visit to begin. This Request for Proposals contains a work statement for performance-based services. This means that the Government has described WHAT is to be accomplished, not HOW to accomplish it, and states a basis for determining whether finished work meets the Government quality requirements. It does not state detailed procedures for accomplishing the work unless there are safety, security, or communication requirements. It is the responsibility of the Contractor to ensure that they build into their quoted price the necessary number of employees and hours it will take to meet the performance standards specified. If any part of the work will be subcontracted, the proposal must reflect the tasks to be performed by the prime and subcontractor, including a breakdown of costs in the price proposal to show that at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. The Contractor will be required to maintain a quality control program to ensure that the requirements of this contract are met. This program shall be created for identifying and correcting deficiencies in the quality of services before the performance becomes unacceptable. Answers to all questions will be posted to FedBizOpps. Proposals are due by 5:00 p.m EST Wednesday, November 19, 2014. Green Cleaning The Contractor shall use green cleaning products and processes and shall demonstrate such capability by submitting a green cleaning plan to the COR that describes methods, materials, and equipment used under the contract. Green cleaning is a planned and organized approach to cleaning that is designed to protect the occupants and workers' health and reduce the impact on human health and the environment. Unlike a traditional cleaning program, a green custodial program takes a holistic approach to building cleaning and goes beyond simple appearances to focus on health and the environmental impacts. Green cleaning products and processes include, but are not limited to products containing recycled content, environmentally preferable products and services, vacuum cleaners with HEPA filtration, bio-based products, and products and services that minimize the use of energy, water, and other resources. In addition to compliance with these requirements, the Contractor shall follow all applicable standard industry practices including, but not limited to those published by the National Institute of Building Sciences (NIBS), American Society of Testing Materials (ASTM), and Carpet and Rug Institute (CRI), as well as applicable standards of the Environmental Protection Agency (EPA). The Contractor shall take every precaution to ensure that if available, only safe and environmentally preferable products are used. Preference shall be given to cleaning products that meet the following: United States Department of Agriculture (USDA) designated bio-based products, Green Seal certified (Stnadard GS-37 for Commercial and Institutional Cleaners, Comprehensive Procurement Guidelines (CPG), and applicable Executive Orders. The success of the green program hinges on the principles of stewardship. Stewardship is the shared responsibility for everyone working together to improve human health and the environment through the Government's cleaning program. The Contractor shall develop stewardship activities that help educate building occupants and present them to the COR. Information on stewardship, training plan, and other issues can be found in ASTM E1971-98, Standard Guide on Stewardship for Cleaning Commercial and Institutional Buildings.
