Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The government intends to issue a Purchase Order in accordance with FAR PART 13.106 for the requirement herein using Simplified Acquisition. The solicitation number is FDA_15-233-1142506. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-77, Oct. 14, 2014. The associated North American Industry Classification System (NAICS) Code is- 325120 - Industrial Gas Manufacturing; Size standards 1,000 in number of employees. The requirement is set aside for small business. The U.S. Food and Drug Administration (FDA), Center for Biologics Evaluation and Research (CBER), is soliciting quotes for weekly delivery of Dry Ice (Solid Carbon Dioxide). Background The FDA/CBER uses Dry Ice for cooling Biological samples while working in the Laboratories and for shipping of Biological samples to other locations. The weekly delivery of the Dry Ice is absolutely critical to the FDA Researcher's ongoing experiments and research. Past usage has been approximately 3,055 pounds of Dry Ice weekly which equates to 12,220 pounds of Dry Ice monthly, 73,320 pounds for six months, and 146,640 pounds annually. Statement of Need and Minimum Requirements: • Contractor shall arrive Monday of each week between the hours of 8:00 am and 12:00 pm (noon) prepared to deliver 3,055 pounds of Dry Ice, Rice Pellet size, approximately one quarter inch in diameter and varying in length from one eighth of an inch to one and three eights of an inch, to the Loading Dock. • The Contractor shall transfer the Dry Ice from the storage bin on their truck to the storage bins that are provided at the Building 52/72 Loading Dock. • The Contractor shall exchange two empty 500 pound contractor supplied Dry Ice insulated containers for two contractor supplied full 500 pound containers. • If only one container is empty, only one container shall be exchanged and the other container brought up to the full mark. • The Food and Drug Administration shall only pay for the amount of Dry Ice that is actually delivered and not pay for the amount shipped. • In the event a holiday falls on Monday the delivery shall be made on Tuesday of that same week. • The Contractor shall adhere to all Local City, County, State, Federal, DOT, EPA, OSHA, NFPA rules and regulations and standards set forth by these organizations. MSDS Sheets shall be provided upon request. • Dry Ice shall be of Standard Commercial Quality. (Apr 2014) The provision at FAR 52.212-2 Evaluation-Commercial Items (Oct 2014) is applicable to this solicitation. The specific evaluation criteria to be included in paragraph (1) of that provision are as follows: The Government will award a contract resulting from this solicitation to the lowest priced (inclusive of all option years) technically acceptable responsible offeror. Offers will be evaluated on their ability to meet the service requirements provided herein. The lowest priced offer will be evaluated first. If the lowest priced offeror is not technically acceptable, the next lowest priced offer will be evaluated and so on until a technically acceptable offer is determined. Technical acceptability will be determined by review of information submitted by the Offeror which must provide a description in sufficient detail to show that the service quoted meets the Government's requirement and demonstrate successful performance of the product/service requested as stated herein. Price proposed must be detailed and represent the offeror's response to the schedule of items. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). The government is not responsible for locating or securing any information, which is not identified in the proposal. To ensure information is available, vendors must furnish as part of their proposal all descriptive material necessary for the government to determine whether the mice meet the technical requirements. Quotes shall include technical specifications, descriptive material, literature, brochures and other information corresponding to each minimum required item, which demonstrates the capabilities of the offeror. The Provision at FAR 52.212-3, Offerors Representations and Certifications-Commercial Items, (Oct 2014) applies to this acquisition. An offeror shall complete only paragraph (b) of the provision if the offeror has completed the annual representations and certification electronically via https://www.sam.gov/portal/public/SAM/#1. If an offeror has not completed the annual representations and certifications electronically at the website, the offeror shall complete only paragraphs (c) through (o) of this provision. It is the offeror's responsibility to monitor the internet site for the release of an amendment to the combined synopsis/solicitation (if any).
