Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is VA251-15-Q-0025 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 339113, and the size standard is 500 employees. (v) This requirement consists of three (3) line items: 1.KSI-PR-WH-2 - Isolation Barriers - KWICK Screen for FLU Season Barriers for ER and Emergency Preparedness, Size: 10' - 6"W x 6' - 3"H, Retractable, Portable, Printable, Flexible screens, Two sided printing - QUANTITY: 4 2.KSU-PORTA - Portable Freestanding Unit- QUANTITY: 4 3.KSI-Brakes - Extra Brakes on front- QUANTITY: 4 Manufacturer: Kwick Screens USA SCOPE OF WORK: The Contractor shall provide four (4) Isolation barriers to be used in the ER and Emergency Preparedness services. These barriers shall service as protective barriers for the Flu Season to keep patients and staff safe from infection. This is a directive from Infection Control service. Location for final installation of this unit will be at the VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. CONFORMANCE STANDARDS: All services provided under this contract must be performed in conformance with the Federal regulations and VA Ann Arbor Healthcare System policies and requirements. These units need to be portable, flexible (can be moved 90 degrees to the right and 45 degrees to the left), printable (see picture attached - must add "VA Ann Arbor Healthcare System" under the graphic "U.S. Department of Veterans Affairs", retractable, bendable (can bend at any point along its axis to make an instant cubicle), and freestanding. These units must meet all of the requirements as directed by Infection Control for clean ability, flexibility, portability, height requirements, ease of configuration by staff not requiring moving and installation crew to move, reconfigure as needed. The height must be no higher than 18" off the floor. The height to the top should be at least 72" high. It must be bleachable for proper cleaning. Infection Control states: The ER waiting room is a very large area where people gather who may transmit infections, e.g., influenza or other respiratory illnesses, to each other. In the event of a major outbreak, it has been standard infection control practice to separate the sick from the well people through the use of physical barriers. If the facility runs out of clinic rooms, barriers of varying sizes can be set up within the ER waiting room space to help separate patients and limit the spread of disease. These barriers can be easily disinfected, set up, and stored when not in use. PREVENTIVE MAINTENANCE: All preventive maintenance will be done by trained staff or an appropriate designated party after the warranty expires. PARTS: Work under warranty must be conducted within a reasonable amount of time depending on the availability of supplies and material for the Isolation Barriers (4). REPORTING REQUIREMENTS: The Contractor shall be required to report to the Interior Design Section at the Ann Arbor VA Medical Center to log in during normal working hours for delivery of this unit. This check in is mandatory. When the service is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out at the Ann Arbor VA Medical Center and submit the field service report to the Contracting Officer Technical Representative for a signature, during normal working hours. If the Contracting Officer Technical Representative is unavailable, a signed authorized copy of the field service report shall be sent to the Contractor after the work has been reviewed. NOTE: Offerors submitting quotations on Equal items shall provide the Manufacturer's Name, Brand and Part Number for each item providing a quotation on. (vi) To be technically acceptable the products quoted shall be Brand Name or Equal to the above noted items and each additional item shall be comparable to what is offered with these items. (vii) Delivery FOB Destination to VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, FAR 52.209-5DEV; 52.225-25; 52.216-1 (fill in: Firm Fix Price contract will be awarded), 52.232-2, VAAR 852-215-70, 852.233-70; 852.233-71 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (o) of this provision.
