- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. THE DEPARTMENT OF VETERANS AFFAIRS IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. In accordance with FAR 15.201(e), under Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought Notice. The purpose of this announcement is to gain knowledge of potential qualified sources and their size classification (hub zone, 8(a), small, small disadvantage, service disabled veteran owned small business) relative to NAICS Code 532291 (size standard of $30.0 Million). 1. The Department of Veterans Affairs (VA) is seeking potential sources capable of providing Home Oxygen Services as well as Durable Medical Equipment (DME) Services to beneficiaries within Veterans Integrated Service Network (VISN) 19. VISN 19 is comprised of the following VA Medical Centers (VAMCs): Fort Harrison VAMC (Fort Harrison, MT) Sheridan VAMC (Sheridan, WY), Cheyenne VAMC (Cheyenne, WY), Denver VAMC (Denver, CO), Grand Junction VAMC (Grand Junction, CO) and Salt Lake City VAMC (Salt Lake City, UT). Place of performance will be required in beneficiaries place of residence. This will include, but not limited to: hospitals, veteran's residence, or in a nursing home where the veteran resides. 2. VISN 19 has approximately 18,725 oxygen patients. Current oxygen patients per VA site (approximate): Fort Harrison VAMC - 2,900 Sheridan VAMC - 800 Cheyenne VAMC - 2,400 Denver VAMC - 4,350 Grand Junction VAMC - 1,275 Salt Lake City VAMC - 7,000 3. Sources should have the capabilities to provide for 24/7/365 Home Oxygen/DME Services. These services include but are not limited to providing for at a minimum Home Oxygen services, Preventive Maintenance (PM) service on all equipment, at least one full-time respiratory therapist dedicated to each VA Medical Center within VISN 19 (Respiratory therapist visits will include follow-ups for new patients and existing patient follow-up as requested to address equipment or safety concerns) and provide safety and ongoing equipment education every 6 months. Services also include storage and maintenance of all high-pressure cylinders, concentrators, portable concentrators, oxygen refill systems, ventilators, emergency back-up systems. The contractor shall also have the ability to arrange travel for veterans outside of the service area in coordination with other vendors at the point of destination. 4. Below is a list of the six VA Medical centers and the areas (regions) that they oversee broken up into counties that fall under VISN 19. a.Eastern Colorado Healthcare System serves the counties of - In Colorado, the counties of Adams, Alamosa, Arapahoe, Baca, Bent, Boulder, Chaffee, Cheyenne, Clear Creek, Conejos, Costilla, Crowley, Custer Denver, Douglas, Elbert, El Paso, Fremont, Gilpin, Grand, Huerfano, Jefferson, Kiowa, Kit Carson, Lake, Lincoln, Mineral, Morgan, Otero, Park, Prowers, Pueblo, Rio Grande, Saguache, Summit, Teller, Washington and Yuma. In Kansas, the counties of Cheyenne, Greeley, Hamilton, Logan, Sherman, Stanton and Wallace. b. Grand Junction Colorado VA Medical Center serves the counties of - In Colorado, the counties of Delores, Delta, Eagle, Garfield, Gunnison, Hinsdale, Mesa, Moffat, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan and San Miguel. In Utah, the counties of Grand and San Juan. c.Cheyenne Wyoming VA Medical Center serves the counties of - In Wyoming, the counties of Albany, and Laramie. In Colorado, the counties of Jackson, Larimer, Logan, Phillips, Sedgwick and Weld. In Nebraska, the counties of Banner, Cheyenne, Deuel, Garden, Keith, Kimball, and Scott Bluff. d. Salt Lake City VA Medical Center serves the counties of - In Utah, the counties of Beaver, Box Elder, Cache, Carbon, Davis, Duchesne, Emery, Garfield, Iron, Juab, Kane, Millard, Morgan, Piute, Rich, Salt Lake, Sanpete, Sevier, Summit, Tooele, Uintah, Utah, Wasatch, Washington, Wayne and Weber. In Idaho, the counties of Bannock, Bear Lake, Bingham, Blaine, Bonneville, Butte, Caribou, Cassia, Clark, Custer, Franklin, Fremont, Gooding, Jefferson, Jerome, Lemhi, Lincoln, Madison, Minidoka, Oneida, Power, Teton and Twin Falls. In Nevada, the counties of Elko, Lincoln and White Pine. e.Sheridan VA Medical Center serves the counties of - In Wyoming, the counties of Big Horn, Campbell, Carbon, Converse, Crook, Freemont, Goshen, Hot Springs, Johnson, Lincoln, Natrona, Niobrara, Park, Platte, Sheridan, Sublette, Sweetwater, Teton, Uinta, Washakie and Weston. In Montana, the counties of Big Horn and Yellowstone. In Utah, the county of Daggett. f.VA Montana Health Care System serves the counties of - In Montana, the counties of Beaverhead, Blaine, Broadwater, Carbon, Carter, Cascade, Chouteau, Custer, Daniels, Dawson, Deer Lodge, Fallon, Fergus, Flathead, Gallatin, Garfield, Glacier, Golden Valley, Granite, Hill, Jefferson, Judith Basin, Lake, Lewis and Clark, Liberty, Lincoln, Madison, McCone, Meagher, Mineral, Missoula, Musselshell, Park, Petroleum, Phillips, Pondera, Powder River, Powell, Prairie, Ravalli, Richland, Roosevelt, Rosebud, Sanders, Sheridan, Silver Bow, Stillwater, Sweet Grass, Teton, Toole, Treasure, Valley, Wheatland and Wibaux. In North Dakota, the counties of Billings, Golden Valley and Slope. 5. Responses to this RFI should be emailed to ricky.robins@va.gov no later than Friday, October 24, 2014. Responses should include the following information: A.Name and address of company B.Point of Contact C.Large or Small Business (Socio-Economic Category) D.Do you operate under NAICS code 532291? (if not, which NAICS code) E.Capabilities to include at a minimum the following information: 1.Do you currently or have you had past experience providing for home oxygen and DME services for the Department of Veterans Affairs? If so, how many VA Medical Centers did you support concurrently within a single contract? Which VISN? 2.If company does not have current or past experience providing services for the Department of Veterans Affairs do you have current or past experience providing services for private or public managed care, insurance or hospital/health systems? 3.What is the highest number of home oxygen using patients your company has provided services for concurrently? Radius of service? 4.Does your company have experience with supporting usage of oxygen devices capable of self-filling tanks? 5.Does your company have the capability to provide portable concentrators? 6.Does your company have the capability to accommodate travel requests? 7.Does your company have the ability to provide for a registered/certified respiratory therapist full-time as part of the Home Oxygen/DME services you provide? 8.Does your company have the capability to provide the necessary resources (i.e. personnel, facilities, materials, transportation, equipment and communication services) to provide Home Oxygen/DME services within VISN 19? 9.Does your firm have experience providing for Home Oxygen/DME services in rural areas during harsh, cold, inclement weather conditions? 10.Does your company have mechanisms in place for thorough, internal, quality-driven performance improvement activities? F.Which regions within VISN 19 could your company provide service to? G.Would you need to utilize a sub-contractor to provide the service (if yes, how much of the service would be sub-contracted out?) H.What Home Healthcare Accreditation does your company currently hold? 6.
