- Statement of Work This is a combined solicitation/synopsis for commercial items with the intent to award IAW FAR 13.106-1(b)(1) on a sole source basis to Intuitive Surgical (hereafter IS) at: 1266 Kifer Road, Bldg 101, Sunnyvale, CA 94086. This sole source is based on IS holding patent on OEM Repair parts and Service Maintenance. The period of performance for this contract is: November 7, 2014 through November 6, 2015. All responsible sources may submit a bid in the form of a capability statement, proposal or quotation. All bids will be considered by the Agency. It is however the sole discretion of the Government not to compete based upon the responses to this notice. Normally, information received will only be considered for the purpose of determining whether to conduct a competitive procurement. Paper copies of this solicitation will not be available. The closing date for submitting proposals is October 24, 2014 at 8:00AM, Pacific Standard Time. This announcement constitutes the only request for quotations. No further solicitation will be issued. The solicitation number is N00259-15-T-0020. This solicitation incorporates provisions and clauses that are in effect through FAC 2005-75. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at the following web address: http://acquisition.gov. The North American Industry Classification System (NAICS) Code for this acquisition is 811219; Size STD: $20.5. All interested Offerors shall submit quotations electronically by email to sandra.kretschmar@med.navy.mil or by facsimile at 619-532-5596, ATTN: Sandra Kretschmar. E-mail submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions. Quotations MUST be received on or before October 24, 2014 at 8:00AM, Pacific Standard Time to be considered. All other offers shall be considered non-responsive. Naval Medical Center San Diego requests responses from qualified sources capable of providing the following listed items: 001 Da Vinci Robotic surgery system maintenance Quarterly 4 The following lists salient characteristics that must be met: 1. New OEM approved repair parts and maintenance must be used. 2. Service shall be conducted to the statement of work attached to this solicitation The following factors shall be used to evaluate offers: 1) Technical capability (new OEM repair parts and qualified service); 2) Delivery Schedule (requirement date is Nov 07, 2014 - Nov 06, 2015); 3) Past Performance (two letters of recommendation from military medical facilities within the past three years); and 4) Price. When combined, technical capability, past performance and Delivery outweigh price as an evaluation factor. Other qualifications that must be met in order to be considered: 1. The Government will only consider firm fixed-price quotations. 2. Offerors must complete and submit quotations IAW FAR provision 52.212-3. 3. Offerors must be registered in the System for Award Management (hereafter SAM) prior to all awards. The website address for SAM is www.sam.gov. 4. A Data Universal Number System (DUNS) number and cage code is required to register.
