- The Department of Veterans Affairs Topeka Kansas VA Medical Center VAMC located at 2200 Gage Blvd Topeka, Ks 66622, is seeking a single award to a responsible vendor to provide the following services. The Contractor shall provide all parts and labor to provide cable and internet services in accordance with the attached statement of Work. This a small business set aside procurement, all responding offerors must be registered in the System for Award Management (SAM) data base. The NAICS code for this requirement is 515210 and the PSC is D399. All Offerors must submit their proposals electronically to arnold.payne@va.gov by the solicitation closing date 12:00 P.M. CST, on October 20, 2014. Responding offerors should provide specifics on the type of cable services and the internet performance levels for evaluation. All technical questions must be presented in writing to the Contracting Officer Arnold J. Payne, Arnold.payne@va.gov, 913 946-1969. Final questions must be received three (3) days before the proposal due date. STATEMENT OF WORK - CABLE TV Contractor shall provide to the Colmery-O'Neil VA Medical Center, 2200 SW Gage Blvd., Topeka, KS monthly bulk cable television service. A representative of the contractor and a VA designee will make an annual outlet recount during each contract year to verify the active drops at the medical center. If satellite service is provided any and all new equipment must interface with existing wiring and equipment. Satellite dishes must be provided and interface with existing equipment and wiring at no cost to medical center. Contractor required to walk all spaces prior to final bids. Programming must be comparable to existing service to include a minimum of the following stations: LOCATION OF MULTIPLE DROPS: a. There is a total of 150 Cable Service Drops and 8 Internet Service Drops located in Bldg's. 1, 2, 4, 5, 6, and 24, at 2200 SW Gage Blvd. Topeka, Ks 66622. b. There is 1 Cable Service Drop and 1 Internet Service Drop located in Bldg. 269, at 3407 SW 21st St, Topeka, Ks 66604. c. Provide modems, routers, and continuous service on all Internet equipment as requested. DAMAGE: Contractor shall repair any damage to the premises caused by the contractor, it's employees, or the agents. Contractor shall hold harmless and indemnify the Government from and against any and all losses or damages resulting from contractor's installation, maintenance, service, removal or operation of the equipment or any other equipment of contractor's except loss or damage arising from negligent or intentional act or omission of the Government. Government will repair any damage to the equipment caused by the Government, it's agents, or any patients on the premises. The Government will hold harmless and indemnify the contractor from and against any and all loses or damages arising from or with respect to: Any negligence or intentional act or omission of Government or its agents or employees, or any patients on the premises. Any claim, demand, legal proceeding or similar action instituted by any person or entity providing multi-channel video programming or other services similar in nature to the services provided to the premises as of or prior to the date of the contract or its successor or assign. INSURANCE: Contractor shall maintain general liability and property damage liability insurance relating to its activities hereunder. EQUIPMENT: Contractor may install, maintain, service, operate and upgrade on the premises coaxial cable and or fiber optic line, internal wiring, amplifiers, converters and other equipment necessary for the services. The equipment will at all times be owned by, and remain the property of the contractors, whether or not attached to or incorporated in the premises, and the Government will not have or obtain any right, title or interest therein. The equipment does not constitute a fixture of the premises. The Government will not, and will not permit any third party to attach to or use in any manner the equipment or any portion thereof. The Government will have no obligation to service or maintain the equipment. Contractor shall have the right to use other telecommunications equipment or wiring on the premises, regardless of ownership, for the delivery of the services. The Government shall provide without charge, adequate space and electricity for the equipment. CONVERTER BOXES: Contractor shall place converter boxes at identified sites. Government shall be responsible for any loss of or damage to these boxes. TERMINATION: This agreement may be terminated prior to expiration of its term by either party upon thirty (30) days prior written notice the other, in the event of either party being in breach of this contract. PAYMENT: Billing is to be monthly. Invoices are to be fully itemized, listing date of service, origin and destination points, individual dollar amounts, and total dollar amounts. Invoices are to be listed in date of order. Monthly payments are authorized upon a receipt of a properly prepared invoice, certified for accuracy by the COR. All invoices against this contract shall reference the contract number and purchase order number and be submitted to: Engineering Service (T-138) Colmery O-Neil VA Medical Center 2200 SW Gage Blvd. Topeka, KS 66622 Engineering budget office located in Bldg. 6.
