(CHECK ALL THAT APPLY AND COMPLETE) 1Specific characteristics of the material or service that limit the availability to a sole source (unique features, function of the item, etc.). Describe in detail why only this suggested source can furnish the requirements to the exclusion of other sources. The GE Medical Equipment reference above is proprietary to GE. Only E Healthcare can perform the required maintenance and repairs to this equipment. 1A patent, copyright or proprietary data limits competition. The proprietary data is: (If FAR 8.405-6(a)(2)iii before posting. Do not include specific proprietary data. Only mention Existing equipment is Philips brand, which requires the GE company to service and maintain per VA and FSS contract V7979P-6019B. Equipment is proprietary and can only be serviced by GE at this time. 1These are "direct replacements" parts/components for existing equipment. We may require a direct replacement part which will be covered under this new task order contract. 0The material/service must be compatible in all aspects (form, fit and function) with existing systems presently installed/performing. Describe the equipment/function you have now and how the new item/service must coordinate, connect, or interface with the existing system. 1The new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order must not have been previously issued under sole source or limited source procedures. This is a follow-up contract to V797P-6019B. 0An urgent and compelling need exists, and following the ordering procedures would result in unacceptable delays. (5) DESCRIBE WHY YOU BELIEVE THE ORDER REPRESENTS THE BEST VALUE CONSISTENT WITH FAR 8.4 TO AID THE CONTRACTING OFFICER IN MAKING THIS BEST VALUE DETERMINATION: The service contract costs for this equipment have been negotiated and a FSS contract has been established for this work. The FSS contract number is V7979P-6019B. GE are the exclusive representative that can work and program the DMS server for the cardiology department since they are the manufacturer. That said, we cannot receive quotes from other vendors that provide after-market parts, since quality will be at risk, thus jeopardizing reliability of equipment and safety of our patients. This is used in special procedures and vascular and is very sensitive. The price has been determined to be fair and reasonable based on established NAC FSS pricing. (6) DESCRIBE THE MARKET RESEARCH CONDUCTED AMONG SCHEDULE HOLDERS AND THE RESULTS OR A STATEMENT OF THE REASON MARKET RESEARCH WAS NOT CONDUCTED: (7) ANY OTHER FACTS SUPPORTING THE JUSTIFICATION: This equipment is required to safely utilize high-tech medical equipment on VA patients. (8) A STATEMENT OF THE ACTIONS, IF ANY, THE AGENCY MAY TAKE TO REMOVE OR OVERCOME ANY BARRIERS THAT LED TO THE RESTRICTED CONSIDERATION BEFORE ANY SUBSEQUENT ACQUISITION FOR THE SUPPLIES OR SERVICES IS MADE: In the future, we will compete the requirement for new equipment and the resulting service contracts will reflect this order. We will also perform additional market research at the end of the fiscal year to see if there are any updates to the FSS contract.
