Description

Experience and Past Performance Form Tohono O'odham Nation Map SOW SF 1449 Soliciation No. 14-249-SOL-00027 is for the provision of On-Site Orthopedic Services for the Tucson Area Indian Health Service (TAIHS). The TAIHS proposes to make a competitve firm-fixed price contract award for on-site orthopedic services for the following locations: 1) San Xavier Health Center, located in Tucson, Arizona 2) Sells Indian Hospital, located in Sells, Arizona (60 miles west of Tucson) Clinics (4 hours = 1 clinic) shall be provided one a month for a total of twelve (12) clinics per year. The anticipated period of performance is for a base period of ten (10) months beginning December 01, 2014 - September 30, 2015, with options to extend for four (4) additional one-year periods, up to a total performance period of four (4) years and ten (10) months. Due Date for responses are on Thursday, November 6, 2015 at 4:00pm MST.
