Description
This is a Request For Information (RFI): The Department of Veterans Affairs (VA), Strategic Acquisition Center (SAC) is requesting responses from qualified businesses capable of providing Surgical and Medical Lasers for the Department of Veterans Affairs. This RFI is for market research purposes only and shall not be considered an Invitation for Bids, Request for Quotations, or a Request for Proposal. Do not submit a proposal or quote. This market research is issued for information and planning purposes only and does not constitute a solicitation nor does it restrict the Government as to the ultimate acquisition approach. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Additionally, there is no obligation on the part of the Government to acquire any products or services described in this sources sought. You will not be entitled to payment for direct or indirect costs that you incur in responding to this RFI. Any contract that might be awarded based on information received or derived from this market research will be the outcome of a competitive process. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this RFI. The purpose of this RFI is to search for information regarding the most recent technology related Medical and Surgical Lasers to include the following: ARGON YAG Nd: YAG Er: YAG YAG; DPSS DPSS DIODE & DIODE PUMPED GREEN TREATMENT BEAM INFRARED DIODE HOLMIUM C02 MULTICOLOR PHOTOCOAGULATION OPTICALLY PUMPTED SEMICONDUCTOR XeCl EXCIMER FEMTOSECOND & EXCIMER The NAICS Code for this requirement is 334510 - Electromedical and Electrotherapeutic Apparatus Manufacturing. Response Date: 20 October 2014 Market Research Questionnaire for Surgical and Medical Laser Technology Instructions: Please provide a written response to the following: 1. Provide a list of the latest available technological advancements in Medical and Surgical Lasers that can be procured from your company. 2. If possible, provide any relevant literature or capability statements for review. 3. Provide information on how your customers manage standardization of lasers throughout their facilities. 4. Provide information on other types of Surgical and Medical Lasers you have available that have the same purpose or usage comparable to the list of lasers listed in the announcement. 5. Provide a list of lasers your company currently provides to the Veterans Health Administration. 6. In addition to lasers, can your company supply all the necessary safety equipment, i.e., goggles, surge protection, signage, etc.? 7. Is there an independent testing laboratory that will verify the accuracy of manufacturer's specifications? 8. Can you offer, for optional purchase, a customizable Service Contract? Examples, of coverages would include: A.Parts only, including laser tube(s) B.PM only C.PM and parts, including laser tube(s) D.Full service including PM, parts and laser tube(s) E.Call back within 2 hours F.Onsite within 24 hours 9. Are all of your lasers FDA approved? 10. Are all of your lasers compliant with current American National Standards Institute (ANSI)? 11. Are all of your lasers compliant with Association for the Advancement of Medical Instrumentation (AAMI)? 12. Are all of your lasers compliant with the current International Electrotechnical Commission (IEC)? 13. List the FDA approved applications/surgeries/procedures for the lasers you sell. For example: iridotomy; SLT; endovascular cardiovascular procedures, etc. 14. What is the single pulse repetition rate in Hz of each type of laser? 15.
