- Performance Work Statement CBA Additional Instructions to Offerors Pricing Exhibit Evaluation Factors Wage Determination Past Performance Questionnaire THIS IS OFFICIAL NOTIFICATION OF THE ISSUANCE OF A REQUEST FOR PROPOSAL (RFP) The United States Air Force, Wright-Patterson AFB, Ohio 45433-5309 is soliciting for a firm-fixed price (FFP) contract for Dining Facility Services, base period and four (1) year options, which also includes up to a 6 month extension. This is a solicitation for a commercial service prepared in accordance with the format in the Federal Acquisition Requirements (FAR) Subpart 12.6. The North American Industry Classification System (NAICS) for this acquisition is 722310. The size standard is $35.5M. This requirement falls under the Randolph Sheppard Act but is also a 100% for small business set-aside. The Dining Facility Service requirement at Wright Patterson Air Force Base (WPAFB) provides full food service for over 1000 airman daily at the Pitsenbarger and 711th facilities. The contractor shall provide all personnel, supervision, and any items and services, except that which is designated Government Furnished Material or Property, necessary to perform full food services. The contractor shall perform the requirements in the Performance Work Statement (PWS), conform to the professional standards identified in the contract, and follow all applicable instructions and directives. For the Performance Work Statement (PWS) and other documents for this solicitation see addendums and attachments. Solicitation number for this requirement is FA8601-15-R-0004. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76, 25 August 2014. The Government reserves the right to de-scope the solicitation prior to award and will amend accordingly. *** Site Visit *** Wright Patterson AFB will be conducting a Site Visit with regards to this solicitation on Friday, 17 October 2014. The details of this Site Visit are as follows: Date: 17 Oct 14 Time: 1:30 P.M., Eastern Location: Hope Hotel & Richard C. Holbrooke Conference Center, Building #823, Area A, Gate 12 A, Wright-Patterson Air Force Base, Ohio 45433. Attendees: 2 company representatives will be granted access per contractor Participation in this event will be at no cost to the government. All travel, lodging, and transportation arrangements will be at expense of the contractor. Base access and participation in this event must be coordinated with Yelena Kanevsky by 3:30 P.M. EST, 14 October 2014. If your company plans to attend this Site Visit, please contact Yelena Kanevsky at 937-522-4601 or yelena.kanevsky @us.af.mil, or contact Richard Fries at 937-522-4567 at richard.fries@us.af.mil. You will need to provide visitor's name, driver's license number, state of issue, and email. The pass will be e-mailed to the visitor and will require an Adobe Reader to open. Print and bring pass to the site visit. All questions concerning this solicitation need to be submitted no later than 3:30 P.M. EST, 6 November 2014. IMPORTANT NOTICE TO CONTRACTORS: All prospective awardees are required to register at the System for Award Management (SAM) and to maintain active registration during the life of the contract. SAM can be accessed at https://www.sam.gov. Any award resulting from this solicitation will include DFARS Clause 252.232-7003, Electronic Submission of Payment Requests. Section 1008 of the National Defense Authorization Act of Fiscal Year 2001 requires any claims for payment (invoices) under DoD contract to be submitted in electronic form. Wide area Workflow- Receipt and Acceptance (WAWF-RA) is the DoD system of choice for implementing this statutory requirement. Use of the basic system is at no cost to the contractor. Contractors must complete vendor training, which is also available at no cost at http://www.wawftraining.com. Prior to submitting invoices in the production system, contractors must register for an account at http://wawf.eb.mil/.
