This is a solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0752 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 621511. All responsible sources may submit a proposal which shall be considered by the contracting officer. Specialized Pathology Services - Statement of Work Part I: General Information A.Introduction: The requirement is for specialized pathology services to include both technical and professional aspects of highly complex procedures in anatomic pathology. B.Background: The VA Northern Indiana Health Care System (VANIHCS), Fort Wayne Campus is responsible for providing anatomic pathology services to our veteran patients and providers. VANIHCS is staffed with a single pathologist and histotechnician, as well as limited equipment. At times, a diagnosis cannot be established with the routine series of stains and tests available at this facility. Therefore it is essential to have a relationship with a reference laboratory that provides specialized anatomic pathology testing to provide more advanced and/or specialized procedures to assist in the interpretation and diagnosis of surgical and cytologic specimens. C.Scope: To establish a relationship with an accredited Specialized Pathology service to provide highly complex anatomic pathology services. Part II: Work Requirements A.Technical Requirements: 1.The contractor shall have the capability of providing a wide variety of specialized anatomic pathology procedures to include, but may not be limited to: i.Immunohistochemical/Immunocytochemical Testing ii.Flow Cytometry iii.Molecular/PCR iv.FISH v.Immunofluorescence 2.The contractor shall provide specialized anatomic pathology services as requested by the VANIHCS pathologist. 3.The contractor shall have professional staff available for consultation with the VANIHCS Pathologist to assist with interpretation of specialized testing results if needed. 4.Estimated volumes based on historical workload: Annual QuanityCPTDescription 6088342Immunocytochemistry per antibody 188312Special Stains Group 1 for Microorganisms 388313Special Stains Group II All other except stains for microorganisms 488343Immunocytochemistry additional separate antibodies 288360Tumor Immunohistochemistry (Her2/Neu, estrogen receptor, progesterone receptor) 188367In Situ Hybridization - each probe B.Deliverables: 1.Turnaround times for individual tests will be defined in the contractors test menu or electronic equivalent. 2.Unanticipated delays in reporting will be communicated to the VANIHCS laboratory before the expected turnaround time is exceeded. C.Period of Performance: October 1, 2014 - September 30, 2015.
