- PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this request. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The U.S. Environmental Protection Agency is issuing this acquisition as a Request for Quote (RFQ) under solicitation number RFQRT-14-00038. This procurement is being conducted as a simplified acquisition in accordance with FAR Part 13. The National American Industry Classification System (NAICS) code for this procurement is 325120 with a small business size standard of 1000 employees. The PSC Code is 6640. This requirement is an open-market full and open solicitation. This procurement is being conducted under Federal Acquisition regulations parts 12 and 13. The resultant contract will be a firm-fixed price purchase order. STATEMENT OF WORK: The contractor shall provide dry ice pellets/rice for use by the National Health and Environmental Effects Research Laboratory (NHEERL), USEPA. Dry ice is used by NHEERL researchers for the collection and quick freezing of animal tissue samples to be used in biochemical assays and molecular biology techniques. Dry ice is also used on gas intake systems for temperature control and for cooling laboratory equipment and accessories to maintain constant temperature. Containers (2 in A wing; 2 in E wing), with either pellets or with rice pellets, shall be filled or replaced on a weekly basis at each location on the USEPA Main Campus, Buildings A and E. The estimated total quantity of dry ice per week for all locations is approximately 1200 lbs of pelleted and 1200 lbs of rice dry ice. The Contractor shall only bring the containers to full capacity. No dry ice shall be left on top of or around the outside of the containers. Pellets must be of medium size, approximately 1 inch long by ? inch in diameter. Rice pellets are to be of actual size. Blocks of dry ice are not acceptable. The delivery must be made once per week (Tues., Wed. or Thurs.) during normal business hours beginning October 1, 2014. The Contractor shall provide the dry ice for twelve months beginning 10/01/14 with 4 one year options as follows: Base period: 10/01/14 ? 9/30/15 Option I: 10/01/15 ? 9/30/16 Option II: 10/01/16 ? 9/30/17 Option III: 10/01/17 ? 9/30/18 Option IV: 10/01/18 ? 9/30/19 Delivery Documentation/Invoicing Prompt receipt by the Project Officer or designee of all delivery documentation is crucial for the prompt payment of invoices. The documentation should be signed and dated by the deliverer. The Project Officer should be notified immediately of any delivery delays, problems or changes in delivery amount or schedule. Although weekly deliveries are required, monthly itemized invoicing is preferred.
