- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0741 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 711510 and the business size standard is small, requestors shall state their business size on their proposal. This requirement is for one musician for the FY15 Catholic Divine Services chaplain Service at the Northern Indiana VA Medical Center's Chaplain's Office. All responsible sources may submit a proposal which shall be considered by the contracting officer. PERIOD OF PERFORMANCE: The requirement is for a one (1) one-year period, the required period of performance shall be 01 October 2014 to 30 September 2015. TITLE: Organist/Musician SCOPE OF WORK: The Worship Musician will provide music for the weekly Sunday Mass and Holy Days. Additionally, the Worship Musician may be called upon to provide music for other special worship events throughout the year such as Christmas, Easter, Memorial services and other special events/celebrations. This list is not all-inclusive. The Worship Musician will coordinate with the Duty Chaplain to ensure that music and vocals are appropriate for theme of the service for that week. Chaplain Service will provide the worship musician with the schedule of Duty Chaplains. The Worship Musician is required to stay for the entirety of the worship event. The Worship Musician will also provide appropriate music before the worship event starts and at the close of the service. LOCATION OF WORK: These events typically take place on the grounds of Northern Indiana Health Care VAMC. They are generally held in the Social Activities Room, the various locked wards and the Chapel. UNIT OF PERFORMANCE: The unit of performance is for one worship event. QUALIFICATIONS: The Worship Musician is required to be familiar with worship resources for the veteran congregations served. They are also required to maintain proficiency in their skills as a Worship Musician. INFORMATION TECHNOLOGY CERTIFICATION AND ACREDDITATION (C&A) REQUIREMENTS The C&A requirements do not apply, and that a Security Accreditation Package is not required. The worship musician does not require access to the VA IT System. PAYMENT SCHEDULE: The Worship Musician will be paid for each worship service where music services are provided by the Worship Musician. Payment to the Worship Musician will be on a monthly basis and will include payment for the number of worship services provided from the first day of the month through the last day of the month.
