Description
Two-Way Communication Repair for Charlie Norwood VA Medical Center This is a Pre-Solicitation Notice for solicitation number VA247-14-R-1603. The resulting contract is for Two-Way Communication Repair Service for the Charlie Norwood VA Medical facilities in Augusta, GA. This acquisition is issued as unrestricted. The North American Industry Classification System (NAICS) code applicable to this acquisition is 334290, 500 Employees. This requirement will be issued as a Firm Fixed Priced Type contract and awarded to the lowest-priced technically acceptable offeror. Anticipated completion for this project is 180 days after the "Notice to Proceed." Work shall be done by certified Motorola technicians. The Contractor shall provide all personnel, equipment, tools, materials, management, labor, supervision, cable pathways and all other items and services necessary to correct two-way radio communication dead spots at Charlie Norwood VAMC, Buildings 801 and 110. FACTORS TO BE EVALUATED 1. Technical (Written) 2. Quality Control Plan (Written) 3. Past Performance 4. Price (1)Section 1 - Executive Summary The Offeror shall describe technical capability of meeting or exceeding the Government's needs: Plan to correct deficiencies (for Downtown facility, Bldg 801 and Uptown Facility 110) and meet performance schedule. (2)Section 2 - Technical Solution a)Project Organizational Chart and Narrative -Clearly describe the prime responsible firm (or firms if a J/V) and individuals as well as the roles and responsibilities of individuals proposed as consultants and subcontractors. Provide a list of all consultants and all proposed major subcontractors, including telephone number, address and name of contact. b)Clarify intended uses of portions of the site indicated to be available to the contractor elsewhere in this RFP for materials staging, temporary trailer offices, employee parking, and other activities as shown in the design solution material. c)Clarify procedures and proposed solutions for contingencies, differing site conditions, power outages, scheduling conflicts and temporary work stoppage requests (interference with Medical Center operations). d)Demonstrate techniques for management of work coordination with subcontractors and measures/corrective action that will be taken for substandard performance. e)Provide an approach to ensure cost-savings and minimizing issuance of change orders. B. Volume II - Quality Control Plan (Deliverable) (1)Quality Control Plan Synopsis (Written). Provide a description of the Contractor's quality control system. Describe in depth your understanding of our needs, how your company will commit to meeting those needs. Describe your experience in providing similar services as required in the Statement of Work to hospital/veteran/military facilities. (2)Describe your schedule to have all work completed in 180 days or less. (3)The name(s) and qualifications of the individual(s) responsible for performing the quality control inspections, and the extent of their authority. (4)Provisions for recording the results of inspections and for recording corrective actions taken. C. Volume III - Past Performance Document (Written) (1)All offerors must have at least five (5) years of past performance providing the requested services. To be considered relevant and recent, the past performance must be of a similar size and scope of this requirement and must have been within the past five years. Each offeror must provide a list of at least one (1) past performance references with the following information: a) Agency / Company name b) Point of Contact c) Contract Number d) Dates Service/Supply Provided e) Amount of Contract/Order f) Phone number The solicitation will be issued on or about September 22, 2014. The estimated closing date is September 25, 2014. The solicitation will not be mailed out. All interested sources must go to the Federal Business Opportunity (FBO) website at: http://www.fbo.gov and obtain an electronic copy of the solicitation. Offerors are responsible to ensure their proposal package is received by the exact time and date at the specified location for receipt of proposals. In addition, offerors are responsible for continuing to monitor the websites for any/all future Amendments issued, if any. Offerors shall sign copies of any amendment issued and submit with their package. Offerors need to be registered in the Central Contractor Registration (CCR) database at http://www.ccr.gov prior to submission of proposal.
