- LIMITED SOURCES JUSTIFICATION ORDER >$3,000 FAR PART 8.405-6 This acquisition is conducted under the authority of the Multiple Award Schedule Program. The material or service listed in par. 3 below is sole source, therefore, consideration of the number of contractors required by FAR Subpart 8.4 - Federal Supply Schedules, is precluded for the reasons indicated below. Restricted to the following source: Manufacturer/Contractor: Olympus America Inc. 1 The requested material or service represents the minimum requirements of the Government. (1) AGENCY AND CONTRACTING ACTIVITY:Department of Veterans Affairs NCO-6___________________________ 27 Averill Dr.____________________ Hampton, VA 23667 VISN:6 (2) NATURE AND/OR DESCRIPTION OF ACTION BEING APPROVED: This justification provides the rationale for limiting sources for Olympus America Inc Endoscopes. A firm fixed priced delivery order will be awarded on a sole source basis Olympus America, Inc with a delivery date of September 30, 2015. (3)(a)A DESCRIPTION OF THE SUPPLIES OR SERVICES REQUIRED TO MEET THE AGENCY'S NEED: The Greenville MOC intends to lease ten Olympus manufactured Endoscopes through Master Lease Agreement within Federal Supply Schedule (FSS), Contract Number V797P-2065D. (b)ESTIMATED DOLLAR VALUE: REDACTED (c)REQUIRED DELIVERY DATE: 9/30/2014 (4)IDENTIFICATION OF THE JUSTIFICATION RATIONALE (SEE FAR 8.405-6), AND IF APPLICABLE, A DEMONSTRATION OF THE PROPOSED CONTRACTOR'S UNIQUE QUALIFICATIONS TO PROVIDE THE REQUIRED SUPPLY OR SERVICE. 1The material/service must be compatible in all aspects (form, fit and function) with existing systems presently installed/performing. Describe the equipment/function you have now and how the new item/service must coordinate, connect, or interface with the existing system. The Greenville MOC currently uses Olympus Endoscopic processors, monitors and light sources. Endoscopic scopes connect to the processor and only Olympus products can interface with the system. Requisition is to lease versus purchasing the endoscopes, based on VHA Deputy Under Secretary for Health for Operations and Management (DUSHOM) Memorandum "VHA National Standardization of Endoscopes" dated 12/06/2010. No other vendor can fill this requirement without a requirement for the clinic to purchase new light sources, processors, reprocessors, and accessories, which represents a significant financial cost to the clinic. (5) DESCRIBE WHY YOU BELIEVE THE ORDER REPRESENTS THE BEST VALUE CONSISTENT WITH FAR 8.4 TO AID THE CONTRACTING OFFICER IN MAKING THIS BEST VALUE DETERMINATION: If different scopes are leased, the Greenville MOC must replace processors, monitors, light sources, and scopes, which represents a significant duplication of cost to the government. (6) DESCRIBE THE MARKET RESEARCH CONDUCTED AMONG SCHEDULE HOLDERS AND THE RESULTS OR A STATEMENT OF THE REASON MARKET RESEARCH WAS NOT CONDUCTED: The Deputy Under Secretary for Health for Operations and Management has issued a directive for all Medical centers to lease their scopes from the Olympus, Pentax, and Fujinon FSS contracts, based on existing equipment. (7) ANY OTHER FACTS SUPPORTING THE JUSTIFICATION: None. (8) A STATEMENT OF THE ACTIONS, IF ANY, THE AGENCY MAY TAKE TO REMOVE OR OVERCOME ANY BARRIERS THAT LED TO THE RESTRICTED CONSIDERATION BEFORE ANY SUBSEQUENT ACQUISITION FOR THE SUPPLIES OR SERVICES IS MADE: None. This action is a result of a VHA standardization effort.
