NOTICE TYPE: SOURCES SOUGHT 3M GAS STERILZER MAINTENANCE AGREEMENT for FY 15 Lebanon VA Medical Centre, VISN 4 - Lebanon, PA Campus This sources sought notice is for market research purposes only. Please provide information and description of services available. A solicitation is not available at this time, but results from this market research will help identify solicitation provisions, should a solicitation occur in the future. The Lebanon VAMC, located 1700 South Lincoln Avenue in Lebanon, PA requires full service maintenance support for 3M gas sterilizer as prescribed within the statement of work listed below. Responses to this notice must be submitted in writing (via E-mail) referencing the requirement & RFI # and must be received not later than 22 September 2014 at 4:00PM. Email: kelly.byers@va.gov. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, description of item(s) available, DUNS number, address, point of contact and social-economic status verification. To be eligible for consideration, offerors will have been engaged in maintaining/servicing the type of equipment system listed for a period of no less than two (2) years and be capable of presenting certification of such with verifiable references. No allowance will be made for a company not having qualified and certified personnel available to respond with prompt and satisfactory service. Interested parties who do not submit the above information on or before the response due date and time might not be considered for this procurement. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website (http://www.fbo.gov) at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. ? Service/Maintenance Agreement Statement of Work Equipment: 3M Gas Sterilizer EquipmentModelSerialPMI 3M Gas Sterilizer487BGP7216386530-0341 A.Title of Project: Full Service Contract and Preventive Maintenance Inspection on the 3M Gas Sterilizer B.Scope of Work: Shall be in accordance with the following specifications: 1.Full Service agreement with scheduled semi-annual inspections, where equipment is cleaned, checked, and adjusted as necessary. Susceptible components are routinely replaced to guard against failure. All parts included during the term of the contract are provided at no charge. Service plans also include unlimited interim calls, 30 day warranty on parts replaced, and emergency calls. 2.Vendor shall provide all personnel, parts, tools, test equipment, service manuals, computer software and schematics necessary to provide repairs and preventive maintenance procedures on all of the identified equipment during normal working hours, 8:00AM - 4:30PM, Monday through Friday, excluding national holidays. 3.The Vendor's service representative (VSR) shall contact Biomedical Engineering (717-272-6621) extension 5469/5466 to schedule a preventive maintenance inspection or prior to responding to a service call. VSR shall acknowledge receipt of a service call within 4 hours and will arrive on the premises to effect repairs within 24 hours. Equipment repaired by the VSR shall be calibrated and safety tested by the VSR before being placed back into service. VSR shall not deviate from this protocol without the prior approval of Biomedical Engineering representative. 4.The vendor's service representative shall present the field service report to a representative of Biomedical Engineering for an authorized signature, leaving one legible copy for inclusion in the individual equipment's Historical Maintenance Record. Vendor shall also provide Biomedical Engineering with a legible copy of the calibration reports including numerical values of measurements taken. Failure to provide Biomedical Engineering with the field service and calibration reports will result in non-payment of invoices submitted for this service. 5.Scheduled preventive maintenance shall include, but not be limited to, electrical safety testing, lubrication, adjustments, calibration, testing and replacement of faulty parts and parts which are likely to fail at no additional charge, returning the instruments or equipment to the operating conditions specified by the manufacturer, NFPA 99 and VA MP-3 guidelines. The selected vendor shall use original equipment manufacturer (OEM) parts, and shall not utilize reverse engineered or third party parts. A copy of the Preventive Maintenance Inspection procedure (including Qualitative and Quantitative tests) used by the vendor service representative for each item listed on the contract, shall be made available upon request. 6.All work shall be performed by competent, experienced personnel qualified to work on the specified equipment in accordance with good manufacturing practices. Vendor shall provide factory training certificates/competencies for all technicians assigned to service the specified equipment, in accordance with JCAHO Standards. Such certificates/competencies shall be submitted to Biomedical Engineering within 30 days from date of contract award. Failure to provide required documentation will result in non-payment of invoices for this service. 7.The scheduled preventive maintenance shall be as follows: Two (2) visits per year, one(1) in February, and the second visit in August. Vendor has the option to provide a scheduled preventive maintenance service at the same time as an emergency service call. 8.Period of Performance Base YearOctober 1, 2014 to September 30, 2015 Option Year 1October 1, 2015 to September 30, 2016 Option Year 2October 1, 2016 to September 30, 2017 Option Year 3October 1, 2017 to September 30, 2018
