- Clauses and Provisions Materials List Supai SF 1449 The Department of Health and Human Services, the Phoenix Area Indian Health Service (IHS), Phoenix Regional Office, is soliciting quotes for cabinets, plumbing, lumber, and miscellaneous building materials for the Colorado River Service Unit located in Supai, AZ. Attachments: SF 1449 Materials List, Supai Clauses and Provisions Brand names have been specified for most of the items, but you may respond with an equivalent description. The brand name or equal purchase descriptions must include, in addition to the brand name, a general description of the salient physical, functional, or performance characteristics of the brand name item that an equal item must meet to be acceptable for an award. NAICS Code: 444110 SIZE STANDARD: $38.5 Million IAW 52.219-6, Notice of Total Small Business Set-aside (Nov 2011), this procurement is 100% set-aside for small businesses. Offers received from firms other than small businesses will not be considered. A firm, fixed price award is anticipated. Best value will be determined by meeting specifications and lowest cost alternative to the government. Quotes are all inclusive. OFFER SUBMISSION: The offer shall be submitted to the Contracting Specialist via email (preferred) at (elizabeth.thorstad@ihs.gov) or may be hand delivered by the RFQ due date and time to Phoenix Area Indian Health Service, Acquisition Management, ATTN: Liz Thorstad, Two Renaissance Square, 40 N Central Ave, Phoenix, AZ 85004. Offers sent by mail or other delivery services shall arrive at the aforementioned address by the RFQ due date and time detailed on the SF 1449. The offer shall include the completed 1449 (Blocks 23, 24, and 30 a, b, and c), completed attachment ‘Materials List Supai' spreadsheet, completed Annual Representations and Certifications, and acknowledge all amendments (if any) by number. Specifications must meet or exceed the brand name or equal characteristics on the attached Materials List document.
