Description

This combined synopsis/solicitation VA-248-14-Q-2098, for commercial items has been prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation that will be issued. A quotation in response to this notice must be received no later than 6:00 pm EST Monday, September 22, 2014. The solicitation number, VA248-14-Q-2098, is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-056. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the offeror's responsibility to ensure that the quote is received. The applicable North American Industry Classification System Code (NAICS) is 339113, Surgical Appliance and Supplies Manufacturing; the size standard is 500 employees. OFFERORS MUST STATE THEIR BUSINESS SIZE when submitting their offer as this is a Small Business Set-Aside and only qualified offeror's may submit bids. This procurement is a 100% TOTAL SMALL BUSINESS SET-ASIDE. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-056. The following FAR and VAAR provisions and clauses are incorporated into this solicitation document by reference: 52.212-1, Instructions to Offerors-Commercial; 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (of which 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-1, 52.225-13, and 52.232-34 are applicable); and VAAR 852.246-70, Guarantee. Additionally, please include a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. 52.212-2, Evaluation - Commercial Items, and VAAR 852.211-73, BRAND NAME OR EQUAL, is also incorporated into this solicitation. The offered items must meet all requirements set forth in the product specifications/salient characteristics section to be considered a responsive offer. The offeror must meet the requirements of the solicitation and have the lowest cumulative price for all items. If an "equal" item is offered, the offeror must submit DESCRIPTIVE LITERATURE that conclusively establishes product equivalency and technical acceptability and compatibility at time of quote submission. Also, if the offeror is not the manufacturer of the quoted items, the offeror shall submit written evidence at time of quote that clearly verifies that the firm is an authorized distributor and/or reseller of the quoted products and an authorized installer/maintenance provider to ensure proprietary equipment, licensing, and warranties are not voided. The Government intends to issue a firm fixed price contract. Offerors shall submit quotes on solicitation VA248-14-Q-2098. Quotes must be submitted electronically via email to janet.lang@va.gov The Subject Line of your email shall state: "RFQ VA248-14-Q-2098 Quote Submission" The offered items must be for New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Bid must be valid for 30 calendar days after close of solicitation. Registration with the System for Award Management (SAM), pursuant to applicable regulations and guidelines is required prior to award. Registration information can be found at www.sam.gov. The VHA NCO 8 - Bay Pines VA Healthcare System, Bay Pines, Florida requires BRAND NAME OR EQUAL, to the following: LI 001, POWER DRIVEN PATIENT STRECHER, Model No. 1125-000-000E (Stryker Prime Series stretchers, Electric, Zoom Motorized Drive System, 30-inch Patient Surface) See the attached 1449 for additional accessories and Contract Line Items: CLIN 01 through 10. See attachment for the "Salient Characteristics."
