- Statement of Work (SOW) RFQ Cover Sheet Issue Date: 15 SEP 2014 REQUEST FOR QUOTE PROJECT: Level III Advanced Motorcyle Training RFQ CLOSE DATE: 1:00 P.M. PST ON 18 September 2014 REQUIRED PERIOD OF PERFORMANCE DATE: 23-26 September 2014 THIS IS A COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only publication. Solicitation number M00681-14-T-0105 is issued as a request for quotation (RFQ). The solicitation documented and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76 and DFARS Publication Number (DPN) 20140828. This procurement is 100% set aside for small business under North American Industry Classification System (NAICS) Code 611692. Small businesses under this NAICS Code are those employing no more than $7.5 million. The Government intends on issuing a purchase order/delivery order to the quoter that represents the Lowest Price Technically Acceptable (LPTA). The following will be considered when determining technical acceptability: adherence to the salient characteristics and delivery terms. This procurement is for Level III Advanced Motorcycle Training meeting the requirements provided in the attached Statement of Work. Attachments: - RFQ Cover Sheet - Statement of Work All responsible sources may submit a response that, if timely received, must be considered by the agency. Quoter is responsible for ensuring that their submitted quote has been received and is legible. Quoters bear the burden of ensuring that quotes (and any authorized amendments) reach the designated email on time and should allow a reasonable time for e-mail transmissions to be completed. All pages of the quote must reach the office before the deadline specified in this solicitation. The quoter bears the risk of non-receipt of electronically transmitted quotations and should confirm receipt. REQUEST FOR QUOTATION ON THE FOLLOWING ITEMS: CLIN ITEM QTY U/I UNIT PRICE TOTAL AMOUNT 0001 Advanced Level III Motorcycle Training Course. Consisting of a two day course. See attached SOW for details. 2 EA ___________ ________________ All sources must be approved curriculum providers, by HQMC SD, prior to contract award. Contractors seeking/requiring HQMC SD approval as curriculum providers must coordinate with and gain approval through the HQMC SD Traffic Safety Manager. Such curricula will have to be reviewed and accepted by HQMC SD before contract award. HQMC SD must review the curriculum, instructor qualifications, and history/track record in presenting the course, as well as the Safety Record of the contractor. Below is a list of current HQMC SD approved Level III Motorcycle Safety Training and/or Motorcycle Mentor Training Curriculum Providers. The Government is not directing which source offeror's should be utilized (if any) but is simply providing a list of previously approved and acceptable curriculum and sources that meet the requirements.
