- STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Clinical Division Water Filtration and Purification 2. Scope of Work: The contractor shall provide all resources necessary to accomplish full service and repair services to include all labor and parts necessary to maintain the Clinical Division water filtration system in operational condition according to manufacturer's specifications. 3. Background: Pathology and laboratory Medicine services use numerous instruments that required purified/filtered water for optimal operation. The filtration system mitigates bacterial contamination that has the potential of adversely impacting patient test results. We are currently using Evoqua Water Technologies (formerly Siemens Water Technologies). 4. Period of Performance: 1 October 2014 through 30 September 2014. 5. Type of Contract: This represents a "Firm-Fixed-Price" contract. B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. The vendor shall provide full service and repair services to include all labor and parts necessary to maintain the Clinical Division water filtration system in operational condition according to manufacturer's specifications. The contractor shall perform preventive maintenance inspections (PMI) at least quarterly at a time mutually agreeable between the contractor and Contracting Officer's Representative (COR). 2. This contract is a full service contract to include all necessary service calls, five (5) days per week, between the hours of 8A.M. and 5P.M., Monday through Friday, Federal Holidays included. 3. The VAMC Atlanta Clinical Division shall be responsible for making the system available to contractors during normal business hours of 08:00 to 17:00. If there is a need for work to be done after hours, the VAMC Atlanta Clinical Division will make arrangements for the contractor to have access to the system. 4. The contractor must perform preventative maintenance inspections (PMI) in accordance with the manufacturer's instructions and will include, as a minimum, the following: a.Visual inspections of tanks, hoses, and other vendor installed hardware. b.Calibration checks shall be performed as outlined by the manufacturer's user's manual and after the installation of replacement tanks. c.Cleaning and lubrication shall be performed at least quarterly. d.Performance checks shall be performed at least quarterly. e.Tank replacements shall be done as necessary and when bacterial contamination is detected. 5. For repairs or services that will be performed during normal business hours (08:00 to 17:00 Monday through Friday), the Contractor's repairman must report to the Blood Bank Supervisor (Renita Scott) or designee (telephone ext. 2011). After all work is complete, the repairman must report to the Blood Bank supervisor (Renita Scott) and provide a written report of services or repairs performed for each item of equipment and must include a listing of replacement parts. 6. Contractors must respond to service calls within one (1) hour of notification and be on station within twenty-four (24) hours. All work performed must be accomplished in accordance with the manufacturer's instructions. D. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. E. GOVERNMENT RESPONSIBILITIES 1. The VAMC Atlanta Clinical Division shall be responsible for making the system available to contractors during normal business hours of 08:00 to 17:00. If there is a need for work to be done after hours, the VAMC Atlanta Clinical Division will make arrangements for the contractor to have access to the system. 2. Payment will be made quarterly in arrears upon receipt of a proper invoice. F. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 2. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. G. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS The contractor must read and sign Contractor Rules of Behavior. The service representative must also complete TMS courses: 1) VA Privacy and Information Security Awareness Training (VA 10176) and 2) Privacy and HIPAA Training (10203). Service representatives must agree to a background investigation and will be escorted by Laboratory technologists throughout the entirety of their visit. H.
