- Terms and Conditions Pricing Schedule Evaluation Criteria Statement of Work Instructions to Offerors - Addenda Brand name Only Justification As prescribed in FAR 12.603(c) (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number RFP-NIHSC-2014 and is a Request for Proposal. (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-76. (iv) The NAICS code for this RFP is 423450 and with a size standard of 100, and is a full-and-open competition. (v) The attached pricing schedule contains all contract line items for this requirements type contract. (vi) The government intends to award a single award requirements type contract for the items listed. Plans for the award can be found in the attachment entitled "Instructions to Offerors." (vii) Delivery orders will be made within 48 hours of receipt of order, and will be delivered to 16050 Industrial Dr, Gaithersburg, MD, or on occasion 9000 Rockville Pike, Bethesda, MD. (viii) A statement that the provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and addenda to this provision can be found with the attachment "Instructions to Offerors (addenda). (ix) FAR 52.212-2 is not applicable to this requirement. The Evaluation Criteria can be found in the attachment entitled "Evaluation Criteria." (x) Please include a copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. (xi) The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Addenda to this clause are included in the Attachment, "Contract Terms and Conditions." (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. The applicable clauses can be found in the Attachment, "Contract Terms and Conditions." (xiii) A statement regarding additional contract requirements are included in the Statement of Work, Attached to this RFP. (xiv) This requirement has not been assigned a rating by the Defense Priorities and Allocations System (DPAS). (xv) Please submit offers electronically, to the contract specialist, cited below, by Septmber 22nd, 2014, by 5PM ET.
