Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, Oklahoma City VA Medical Center 921 N.E. 13th, Oklahoma City, OK 73104 has a requirement for radiopharmaceutical products and delivery to the VA on date and time specified when the ordered is placed. Contractor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals for at least three (3) years. The NAICS code is 325412, and the size standard is 750 employees. Request for Quotation (RFQ) is issued under number VA256-14-Q-1582. There is no set-aside requirement for this procurement. The period of contract will be 10/01/2014 through 12/31/2014. The Government's intent is to make a firm-fixed price contract to the responsive and responsible Lowest Price Technically Acceptable offeror. All interested bidders shall submit quotations electronically by email to lakeisha.white@va.gov Telephone replies will not be honored. Bidders should supply a point of contact, company address, phone, fax and email address and must be registered in the SAMS database (www.SAMS.gov) with no debarments or suspension (EPLS) to be considered responsive. The bidder should confirm receipt of email submissions. Quotations are due by September 22, 2014, 4:00 PM, Central Standard Time to be considered responsive
