Description

The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue Lebanon, PA 17042, has a requirement for a full service commercial dental laboratory. The laboratory shall be able to provide chair side consult upon request. The dental laboratory shall have the appropriate licenses and certifications required to provide this service. The required licenses and certifications will be posted with the solicitation. The contractor shall perform all laboratory steps required to create and provide high quality dental prostheses. The turnaround time shall be three (3) to five (5) days, to include pickup and delivery. It is expected that between five (5)) and ten (10) weekly inside pickups and/or inside deliveries to Lebanon VA Medical Center will be required. Point of pick up/delivery at Lebanon VA Medical Center will be specified upon award. The particular times for scheduled pickups will be determined by mutual agreement between the VA Dental Service and the Contractor. Primary place of performance is the Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042. Period of performance will be one year from the date of award, with two options periods if determined to be beneficial to the Government. The Government intends to issue a Request for Quote (RFQ) and award a Firm-Fixed-Price (FFP) contract to the responsible Offeror whose quote constitutes the lowest price technically acceptable. This acquisition will be competed amongst small businesses that can provide the service described above. The North American Industry Classification System (NAICS) for this requirement is 339116 with a small business size standard of 500 employees. This contract will be based on the availability of funds. All interested and responsible parties may submit a quote following solicitation release. The solicitation number is VA244-14-Q-1592. It is anticipated that the solicitation will be posted to the Federal Business Opportunities website on or about the week on or about the week of October 1, 2014. Offerors must be registered in the System for Award Management (SAM) database (http://www.sam.gov) and complete online Representations and Certifications at (http://www.sam.gov). See FAR Clause 52.204-7.
