RFP VA247-14-R-1636 Combined Synopsis/Solicitation for Medical Services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. A)Solicitation number VA247-14-R-1636 is being issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clause are those in effect through Federal Acquisition Circular 2005-75. The solicitation is a total Service Disabled Veteran Owned Small Business set aside and associated NAICS 621111, small business size standard of $11.0 million. All proposals are due on B)Description of Requirement (Performance Work Statement is Attachment #1): The Contractor shall propose two (2) board certified Internal Medicine or Family Practice physicians key personnel to be credentialed and be available for scheduling to meet the requirements of the contract. Contractor will be required to complete Vetpro submission within (7) seven days of award. Solicitation responses must include statement which clearly specifies offeror is capable of submitting credentialing package into Vetpro system within (7) days of award. Submission of credentials in the VetPro Credentialing System is required of all VHA licensed, registered, and certified health care providers. This system is used nationwide at all VHA healthcare facilities. Additional information regarding Vetpro can be found at this site: http://www.vacareers.va.gov/job-search/downloads/VetProGuide.pdf ii.Solicitation response submittals should include the following: 1-Completed CLIN schedule with pricing, physician name and title/level of experience. Additional names of certified physicians may also be submitted, but are not required. 2-Capabilities statement of offeror submitting proposal including statement from section (i.) above and emergency replacement plan. The emergency replacement plan is the backup plan to provide alternate physicians in the event of emergency staffing replacement as needed, such as physician departs from position, takes emergency leave of absence, etc. 3-Past performance history-Reference list of up to (3) three prior healthcare personnel service contracts completed. Each reference must include period of performance dates, contract/purchase order number, total dollar value of contract, details of types/specialties of healthcare personnel provided to entity and a brief description of services performed and entity's contract administrator point of contact name, telephone number and email address. D)The provision at FAR 52.212-2 Evaluation-Commercial Items (Jan 1999) applies to this acquisition as follows: i.(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered, using Lowest Price Technically Acceptable procedures. Offers are considered technically acceptable upon submittal of all items listed in instructions to offerors, using evaluation factors listed below based on completeness of submission. The intent of the government is to award without discussions.
