Fitness Kiosk Solution Attachments 1 - 8 RFQ Documentation #F41999-14-Q-0544 This Request for Quotation (RFQ) is issued by the Air Force Nonappropriated Fund Purchasing Office (AFNAFPO). All requirements, solicitations, and subsequent contracts, if any, awarded through AFNAFPO are funded wholly with Nonappropriated Funds (NAF). These funds are generated through Air Force Morale, Welfare, and Recreation (MWR) programs worldwide. THEY ARE NOT APPROPRIATIONS FROM CONGRESS OR "TAXPAYER" DOLLARS. This is a Request for Quotation (RFQ): The Air Force Nonappropriated Purchasing Office (AFNAFPO) is seeking vendors to support the following to provide a fitness kiosk solution that supports the healthy base initiative. The overall objective of this initiative is to provide 24/7 access to fitness opportunities by utilizing a useful, readily accessible solution to better equip service members and eligible patrons to meet their fitness goals. The fitness kiosk solution is a multitude of virtual fitness instructional classes designed to increase user resiliency and fill training voids in the existing fitness programs. We are looking for a vendor to provide the necessary hardware, software and firmware for a fitness kiosks solution; to include all equipment, installation, and systems operational training for forty (40) designated Fitness Centers worldwide. The objective of this RFQ is to award a single, firm fixed price, Nonappropriated Fund Purchasing Agreement (NPA) to the Offeror with the fitness kiosk solution that yields the lowest price technically acceptable (LPTA) solution with acceptable past performance. The successful Offeror must be able to implement the fitness kiosk solution to DoD Fitness Centers worldwide in accordance with the Statement of Work in Attachment 2. The following RFQ documentation must be submitted for consideration for award. Please ensure all requested documentation is submitted on time. a. Complete blocks, 6, 7, and 10-16 of the Nonappropriated Fund Request for Quotation Form b. Potential Project Schedule c. Completed Group Fitness Kiosk Platform Pricing Schedule d. Technical Solution and Narrative outlining a completed understanding of the requirement and how contractor will fulfill all requirements of the Statement of Work (SOW) e. Completed Representations and Certifications f. Two (2) narratives outlining completion of similar efforts matching the size and scope of this requirement. The NAFI will evaluate the quotes based on the technical responses (pass/fail) and price. The NAFI will award a Nonappropriated Fund Purchase Agreement (NPA) to the vendor whose quote represents the lowest price, technically acceptable solution. The base performance period for the NPA is two (2) years with eight (8) one (1) year options. Reference attachment to this letter for format only; an actual NPA will be completed after the LPTA vendor is chosen. Quotes must be received by the Contracting Officer NO LATER THAN 4:00 PM CST, Monday, 13 October 2014. Incomplete quotes impede the evaluation process and will not be accepted. Quotes in response to this RFQ may be submitted electronically (via email) to: AFSVABusinessOperations@us.af.mil. Questions regarding the RFQ must be submitted in writing to the Contracting Officer by: 4:00 PM, CST, Monday, 22 September 2014. Questions may be submitted via email to: AFSVASVCBusinessOperations@us.af.mil The Contracting Officer will respond to all questions received in a timely manner no later than 4:00 PM, CST, Wednesday, 01 October 2014 (7 Business Days).
