***SPECIAL NOTICE**** THIS IS A CORRECTION TO THE PREVIOUS NOTICE SUBMITTED ON SEPTEMBER 02, 2014. THIS NOTICE IS A REQUEST FOR PROPOSAL (RFP); FOR ILUMIENA PCI Optimization System Media Kit; The Birmingham, Alabama VA Medical Center is currently seeking the equipment to support the EP/CATH Lab. Items shall be Brand Name of Equal in accordance to the terms and conditions of FAR 52.211-6/ VAAR 852.211-6. (1)OFFERORS SHALL PROVIDE ALL REQUESTED DOCUMENTATION LISTED IN THE SF1449 DOCUMENT LOCATED BELOW TO BE CONSIDERED FOR AWARD (2)PROPOSALS SHALL COMPLY WITH THE TERMS AND CONDITIONS OF THE ATTACHED SF1449 DOCUMENT. (3)ONLY SMALL BUSINESSES SHALL APPLY (4)SOLICITATION HAS BEEN EXTENDED UNTIL 4:00P.M. EASTERN TIME ON 16 SEPTEMBER 2014. Submission Requirements: Responses to this RFP must be received no later than 4:00 p.m. Eastern Standard time, Tuesday, September 16, 2014. Offerors shall submit documents via email only to millicent.ball@va.gov.
