Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. RFQ VA263-14-Q-5289 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 05-76. The applicable NAICS code is 334513. The requirements of this solicitation are one (1) The microSTAR Ž ii Medical Dosimetry System is a simple, flexible solution to independently verify planned dose to patients in radiation oncology. The microSTAR Ž ii system includes a microSTAR Ž ii reader, computer, software, bar code scanner, power adapters, USB cable, set of calibration dosimeters, set of QC dosimeters, and electronic version of the user manual (pre-loaded on the laptop). Exposure type available for the calibration and QC dosimeters - 662 keV beam quality (Cs-137) and Fifty (50) Screened nanoDotTM Medical Dosimeters (Item: 18100-000) Screened nanoDotTM Medical Dosimeters have been tested by Landauer to provide accuracy of +/- 5% (total uncertainty - single measurement). Please provide quote for the requested items or the equivalent to the specifications of the requested items. Include in your quote the following information: (a) Installation (if applicable); (b) Training (if applicable); (c) Warranty (parts and labor); (d) Estimated shipping and handling; (e) Delivery (ARO); and (f) Availability of service and operator's manual(s). Also, please include GSA Contract # (if applicable), DUNS #, Tax ID number and fax number. All quotes submitted in response to this RFQ must remain firm through - October 31, 2014.
