Description
Tucson OR Doors Replacement Statement of Work Title: Replace the doors in the Operating Room (OR) department at the Southern Arizona VA Health Care System (SAVAHCS) in Tucson, AZ. Description: The contractor shall remove and replace approximately thirty-four (34) doors, associated hardware, and frames at twenty-five (25) different locations in building 57, 2nd floor. The current doors need to be replaced, which involves the removal of the existing doors, the installation of new hinges and other hardware, and the installation of new doors. Requirements: Contractor shall remove and replace doors, frames, and all associated hardware, in a manner to mitigate noise, dust, and flying debris to the greatest extent possible. Contractor shall also maintain a safe access and egress for patients, staff, and visitors at all door locations during the entire course of work. Contractor shall notify the COR in writing fourteen (14) calendar days, prior to any work activities and provide a removal and replacement plan for the removal and replacement of any doors, frames, or associated hardware. Item 1 and item 2, on the attached building 57 door schedule, are high traffic, sensitive areas and shall require coordination and a plan submission for COR review and approval. Contractor shall provide new 1 ¾'' Acrovyn [eiDOOR (Eggers Industries) or equal]. The products shall meet or exceed the below specifications: "Solid core, flush, high impact resistant, non-rated wooden "Lead lined doors shall have a minimum of an 1/8'' inch lead line. "New door frames shall be a minimum of 16 gauge. "All Door faces and edges shall be PVC-Free high impact resistant engineered Acrovyn 4000 series #336 Red Oak or Wilsonart stained red oak VA match, or equal. "Edges shall be replaceable and ¾'' thick. "All associated hardware shall be as per attached building 57 door, frame, and associated hardware schedule. "New light kit heights, dimensions, and aesthetics, shall match that of existing light kits. "ALL new doors and new door areas shall have a minimum of a 20 minute fire rating. "New doors are NOT required to have any kick, mop or protection plates. Definitions: Define and spell out all acronyms. "SAVAHCS - The Southern Arizona VA Health Care System is the health care system in the Tucson, AZ area. Also in this SOW, it refers to the main VA Medical Center in Tucson, AZ (not the surrounding clinics and other facilities). "POP - Period of performance. "COR - Contracting Officer's Representative Place of Performance: This project encompasses twenty-five (25) different locations in the VA Medical Center in Tucson, AZ in building 57 on the 2nd floor. "Tucson VA Medical Center: 3601 S. 6th Ave. Tucson, AZ 85723 Period of Performance: The removal, delivery, installation and clean-up shall be completed within 120 days after the date of award. The warranties shall begin from the date the last unit is operational. Installation Schedule: The contractor shall perform the removal and installation and will not have access to patient sensitive information. Installation shall take place during regular business hours (e.g. 8 am - 4:30 pm). Removal and Disposal: The contractor shall be responsible for the removal and disposal and the cost shall be included in the line item pricing. Contractor shall allow the VA to remove all desired hardware, lights, plates, and any other desired material from existing doors prior to the proper discard of existing doors by the Contractor. The contractor shall replace or repair any damage caused by the contractor to the Government's property (i.e. walls, floors, ceilings, doors, existing frames, hospital equipment, etc.). Contractor shall provide the manufacturer recommended spare parts to the VA for any door accessories or hardware. Clean-up: Clean-up is required after each door is installed and operational. Acceptance: Acceptance will be after installation and clean-up is complete in all areas and inspected by the COR. Security: No VA sensitive information is included in this requirement. The contractor must check in with the authorized government representative and obtain badging as necessary. On occasion there may be a requirement for escort services provided by the VA, i.e. (patient care areas, pharmacy, IT, etc). COR: Nathan Scott is nominated as the Contracting Officer's Representative (COR) to help oversee the day-to-day operations, especially during the removal, installation and invoicing/payment procedures.
