- Statement of Work for Requiremnt This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation Number. FA4800-14-T-0004; Request For Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-76 August. 25, 2014. This is a full and open competitive RFQ. The NAICS is 511220 with a size standard of $35.5M. The following is a list of contract line item numbers and items, quantities and units of measure, including options. The contractor shall furnish all labor, materials and equipment to deliver custom Emergency Room Patient Encounter Templates/Forms and Licensing Agreement for the Emergency Medicine Paper Charting System for the United States Air Force (USAF) Hospital, the 633d Medical Group at Joint Base Langley-Eustis (JBLE), Virginia (VA) subject to the terms and condition of the contract. The following specifications apply to this project. The project site is the Emergency Room at the USAF Hospital Langley for the 633d Medical Group located at Joint Base Langley-Eustis, Building 257, 77 Nealy Avenue, Langley AFB, Virginia.
