- 1.General Guidance a.Title of Project: Transportation of remains from New York Harbor Healthcare System (VA NYHHS) - Manhattan Campus to James J. Peters Veterans Affairs Medical Center (Bronx), VA NYHHS Brooklyn Campus or St Albans Community Life Center (CLC) b.Background: Due to the events of Hurricane Sandy, the VA NYHHS - Manhattan Campus' morgue was completely destroyed. Construction has commenced and there is no morgue available to hold remains until proper procedures and transportation have been arranged. c.Scope of Work: The contractor must have a current NY State Funeral Director's license, and preferably be a funeral home. The contractor must obtain a NYC transit permit for for the transportation of the deceased veteran, and follow NYC and NY State Sanitary Code in the transportation of the deceased. The transportation of remains will commence from VA NYHHS - Manhattan Campus to a designated location (Bronx VAMC, Brooklyn Campus, or St Albans CLC) upon contact from the Manhattan Campus morgue office. The contractor will have two (2) hours to retrieve the remains and transfer to the designated location. The contractor, upon request, shall provide quality data and information related to services for participation in VA NYHHS Performance Improvement Programs when requested to do so. Upon request the contractor shall also provide documentation of the competency of staff that shall be providing the contracted services. The Contractor shall ensure Contractor employees providing work on this contract are fully trained and completely competent to perform the required work; they shall also maintain records that document competence/performance level of employees working on this contract. Upon request, the contractor shall provide a current copy of their competency assessment checklist and annual performance evaluation to the Contracting Officers' Representative (COR) for each contractor employee working on this contract. The contractor shall, at all times during the contract period, have on his/her premises a functioning fax machine, email and telephone to communicate requirements with the VA (i.e., incidents, requests for service, special correspondence, etc.). Each contractor shall indicate if there is e-mail, fax and telephone availability for their company to complete communications with the VA (use of this communication system shall be mutually agreed upon). The Contractor shall maintain at a minimum, a 95% compliance rate on response to pick-up or deliver during the contract year. If the Contractor fails to provide service within the time frames specified herein, the Government reserves the option of either obtaining service from an alternate source and charging the Contractor with any excess costs, or assessing a fee for each late pick-up. Compensation will be obtained by deducting the applicable amount from Contractor's invoice as specified below: Minutes Late Deduction 15 to 30 25% of trip value* 31 to 45 50% of trip value* 46 to 60 75% of trip value* 61 or more Trip shall be at no cost to VA The Contractor shall submit an invoice summary that includes the contract number, purchase order obligation number, invoice date, invoice number, dates of service(s), total amount due and billable rates per the price and cost schedule. Transportation services listed in this Performance Work Statement do not include ferry, bridge, or road toll charges. Any legitimate toll charges shall be limited to ONE WAY ONLY and shall be listed separately on the Contractor's invoices. The contractor will be available to transport remains within two (2) hours of contact. The contract will remain in effect for one year from date of award plus one twelve (12) month option year if necessary. f.Type of Contract: Indefinite Delivery, Indefinite Quantity (IDIQ) 2.Contract Award Meeting a.A post-award meeting will be held so all parties will understand the needs of the Government. At this meeting the Contractor is required to discuss with the VA NYHHS: i.Pertinent personnel contact information ii.Provide make, model and license of vehicle(s) iii.Provide insurance information 3.General Requirements a.Provide transportation of decedent within two (2) hours from notification from Manhattan VAMC to one of the following locations; Brooklyn VAMC, Bronx VAMC or St Albans CLC b.Abide by all city, state, and federal laws pertaining to the transportation of remains c.Contractor must be available 24 hours seven (7) days a week. d.Contractor may require a body bag in transport of veteran. 4.Evaluation Criteria a.Past Performance: b.Comparative Price from other government agencies, private and public medical centers which use similar services c.Quality Assurance: minimal failure of on time pickups and drop-offs of decedent d.Technical Capability to provide the required service to meet the needs of the VA NYHHS 5.Reporting Requirements a.Contractor will provide time of pickup and drop-off for each delivery of remains. b.Any accidents, delays must be communicated immediately to the COR and/or designated individual 6.Confidentiality and Nondisclosure a.Contractor employee(s) must fill out VA Form 0752, Confidentiality of Sensitive Information Non-disclosure Agreement 7.Contractor Personnel Security Requirements: No security requirements required
