- This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and quotes are being requested. A written solicitation will not be issued. The solicitation number is VA248-14-Q-2031 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219 with a small business size standard of $20.5M. This solicitation starts 09/09/2014 and will end 09/23/2014 at 14:00 pm EST. This solicitation is a Meet or Exceed and is unrestricted with preference to small business determined to be technically acceptable and in the competitive range. The Department of Veterans Affairs Bay Pines Healthcare system, C.W. Bill Young Veterans Affairs Medical Center, hereafter referred to as CWYVAMC, 10,000 Bay Pines Blvd. St. Petersburg, FL requires technical and professional services for inspection and repair of Drager Medical Anesthesia Units as identified below for the CWYVAMC Biomed Department. This is a Firm Fixed Price, Base Year with four (4) one year options requirement. Period of Performance Base Year: 1 Oct 2014 - 30 Sep 2015. SCOPE OF WORK: The CWYVAMC at Bay Pines, Florida requires technical and professional inspection and repair service for Drager Medical Anesthesia Units listed below: 1.Anesthesia Unit, Model Fabius GS, EE# 42241 2.Anesthesia Unit, Model Fabius GS, EE# 42245 3.Anesthesia Unit, Model Fabius GS, EE# 42243 4.Anesthesia Unit, Model Fabius GS, EE# 42246 5.Anesthesia Unit, Model Fabius GS, EE# 42249 6.Anesthesia Unit, Model Fabius GS, EE# 42244 7.Anesthesia Unit, Model Fabius GS, EE# 42248 8.Anesthesia Unit, Model Fabius GS, EE# 42242 9.Anesthesia Unit, Model Fabius GS, EE# 42247 10.Anesthesia Unit, Model Fabius Tiro, EE# 51081 11.Anesthesia Unit, Model Fabius Tiro, EE# 90861 General: The contractor shall provide all repair parts and service for maintenance on the Draeger Medical Anesthesia units located at the C.W. Bill Young VAMC, Bay Pines, Florida, 2D Operating Rooms. Concentration verification of vaporizers (all vaporizers used with the Draeger Units) is every six months. Repair of vaporizers is not included in this agreement Hours of coverage will be from 8:00 am through 9:00 pm Monday through Friday excluding Federal Holidays (New Years Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day). All work done outside the normal coverage time should be first approved through the Biomedical Engineering Section, VA Bay Pines. Preventive maintenance should be negotiated, during normal hours, at least one month in advance with the Operating Room Nurse Manager or his or her designee. Preventive maintenance shall be done at the manufacturer's recommended frequency. Hardware and Software: 1.Hardware and software upgrades will be scheduled and performed during normal working hours, at no additional charge to the VA. These upgrades are those required for the correct function of the system; not those purchased to enhance function. 2.The contractor shall provide all hardware and software necessary for unscheduled repairs. The contractor's field service engineer shall respond by phone within 2 hours and respond on site within 48 hours to correct the problem. Uptime requirements, excluding scheduled preventive maintenance, shall be 97% of the normal working hours and working days during the term of the contract. All broken parts replaced, to repair the system, shall become the property of the contractor. Inspection and Repair: The contractor shall have trained personnel within a 300 mile radius of Bay Pines, Florida, with an adequate spare parts inventory in some area office and proof of manufacturer's or equivalent maintenance training of field service personnel. Each scheduled and unscheduled visit shall result in the production of a written report, by the contractor's representative, in a format negotiated between the VA and the Contractor. The contractor shall always contact the Biomedical Engineering Section Room# 2D200-100, (727) 398-6661 extension 5601 (after hours, extension 7777) before and after arrival at the site Failure to provide the information in the above paragraphs shall result in your quote being determined "non-responsive". The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items.
