Solicitation Number: VA257-14-Q-1707 Notice Type: Combined Synopsis/Solicitation Synopsis: 09/05/2014 The Department of Veterans Affairs, North Texas Veterans Health Care System (NTVHCS), at 4500 S. Lancaster Rd. Dallas, Texas 75216, has a requirement for Courier Services. The contractor shall furnish all labor, tools, materials, equipment, and supervision necessary to provide same day courier services for the VA North Texas Health Care System Community Based Outpatient Clinics (CBOC's): Denton CBOC, 2223 Colorado Blvd, Denton, TX 76205 Greenville CBOC, 4006 Wellington Rd., Suite 100, Greenville, TX 75401 Granbury CBOC, 601 Fall Creek Hwy., Granbury, TX 76049 VA Fort Worth Outpatient Clinic, 2201 SE Loop 820, Ft. Worth, TX 76119 VA Polk Street Annex, 4243 S. Polk Street, Dallas, TX 75224 Other locations as needed on an intermittent basis and deliver to the Pathology & Laboratory Medicine Service (P&LMS) laboratories located at: Dallas VA Medical Center, 4500 S. Lancaster Rd., Dallas, Texas 75216 Sam Rayburn Memorial Veterans Center, 1201 E. 9th Street, Bonham, Texas 75418 Other sites may be added or deleted as need arise. The NTVHCS Dallas is contemplating a firm-fixed price contract. The proposed contract will have a twelve-month base period, on or about 10/1/2014 through 9/30/2015, with four optional one-year provisions. The NAICS code is 492210/size standard $25.5M applies. The VA will award a contract resulting from the Request for Quotations (RFQ) to the responsible offeror whose offer conforming to the RFQ will be most advantageous to the Government. The following factors will be used to evaluate quotes: Price, Technical, and Past Performance. Electronic copy of the solicitation is available by email at victor.hinojosa@va.gov. Offerors are advised that they are responsible to contact the office to register for the Bidders Mailing List in order to receive notification of any Amendments. For obtaining amendments, it is essential that offerors register on bidder's mailing list for this solicitation to receive notification of amendments.
