Description

The Contractor shall provide the following Adult Day Health Care (ADHC) Services to all eligible veterans upon referral by the Chief, Extended Care Service Line, or his/her designee, of VA Northern Indiana Healthcare System. The Contractor shall provide all necessary personnel, equipment, and supplies associated with the full range of ADHC services in performing the requirements of this contract and as outlined in Statement of Work (SOW). Contractor's facility shall be located in the Primary Service Area of VA Northern Indiana Healthcare System, in order to service veterans residing in the area.
