Description
(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)The solicitation number is: VA259-14-Q-0727, and it is issued as a request for quotation (RFQ). (iii)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74. (iv)This solicitation is not a set aside. Nature and/or Description of the Action Being Processed: Surgical Services at the Cheyenne VAMC is requesting two Zeiss Visual Field systems be purchased. The visual fields requested are replicas to the current visual fields that the Cheyenne VAMC has on site. The two new visual fields will be placed in the Eye Clinic at the Cheyenne VAMC as well as the Greeley MSOC. Having exact model visual fields will standardize the equipment, making operation for eye clinic employees standard on all machines. 3.Description of Supplies/Services Required to Meet the Agency's Needs: Two complete visual field systems Required Delivery: Earliest Possible or by 10/30/2014 Items: "Zeiss O/S HFA 750i w/talbe including USB, Dicom Gateway, 2.0, DG 2.0 License, NETPRO GPA, Sita Swap & Kinetic Specifications: Visual Field systems requested will reduce training time, because it is in addition to existing equipment already in possession at the Cheyenne VAMC. 4.Statutory Authority Permitting Other than Full and Open Competition: Cheyenne VAMC is already in possession of one Zeiss Visual Field system. The goal is to provide standardization throughout the clinics and the CBOC's. The addition of two more visual fields would provide standardized equipment as well as standard operating procedures for all eye clinic staff. 5.Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): The Cheyenne VAMC is requesting two visual field systems to provide more availability in our scheduling in the Cheyenne VAMC Eye Clinic and expand our services to the Greeley MSOC. The Cheyenne VAMC currently has one Zeiss visual field system on station. Accordingly, two more Zeiss visual field systems would provide standardized equipment in all locations and would provide standardized operating procedures for all eye clinic staff. 6.Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: Product must interface with existing visual field equipment. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable 8.Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Market research was performed in finding these visual field systems. Currently, we have one Zeiss visual field system on station. We are requesting two more to provide more availability in scheduling our eye clinic appointments and expanding our services to the Greeley MSOC. 9.Any Other Facts Supporting the Use of Other than Full and Open Competition: None, see paragraphs above 10.A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: None.
