Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-14-Q-0888 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-76 effective 08/25/14. The associated North American Industrial Classification System (NAICS) code for this procurement is 339114 with a small business size standard of 500 employees. This requirement is: an unrestricted, Brand Name or Equal, Meet or Exceed. FOB Destination shall be Phoenix, AZ 85012-1892. The VHA NCO 18 - Phoenix, AZ requires the following items, Brand Name or Equal, Meet or Exceed, Lowest Price Technically Acceptable: DENTAL EQUIPMENT AS DESCRIBED IN THE ATTACHED STATEMENT OF WORK. REFER TO B.3 PRICE/COST SCHEDULE IN THE SOLICITATION FOR ALL LINE ITEMS. VENDOR SHALL PROVIDE A PRICE QUOTE AND EXTENDED TOTALS, FOR ALL LINE ITEMS AS LISTED IN B.3 PRICE/COST SCHEDULE; VENDOR SHALL ANNOTATE PRICES OFFERED FOR EACH LINE ITEM ON THE B.3 PRICE/COST SCHEDULE.' ***Question Submission: Interested offerors shall submit all questions concerning the RFQ at the earliest possible time to enable the Buyer to respond. Questions must be submitted via email to: Bryan.Rogers2@va.gov. Questions not received within a reasonable time prior to close of the RFQ may not be considered. *** Failure to comply with the terms and conditions as stated in this announcement may result in offer being determined as non-responsive. New Equipment ONLY; NO remanufactured or "gray market" items. All items shall be covered by the manufacturer's warranty. Quote SHALL be good for 60 calendar days after due date of quotations. This solicitation requires registration by the vendor with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. The selected Offeror shall comply with all clauses and provisions as listed in the RFQ.
