Description
Background and Objectives The U.S. Department of Veterans Affairs (VA) is seeking a Contractor to assist the James A. Haley Veterans' Hospital (JAHVH) with the provision of housing placement and case management services through the HUD-VASH program. The program is a partnership between the U.S. Department of Housing and Urban Development (HUD) and the VA Supportive Housing (VASH) program to place and maintain homeless Veterans in permanent, community based housing using HUD Section 8 Housing Choice Vouchers that are paired with ongoing VA case management services. Many of these Veterans have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. VA intends to engage a Contractor to provide housing placement services and on-going case management to VA referred Veterans and their families. Entities that are interested in providing these services should be aware that HUD-VASH is a national program serving Veterans and their families throughout the country. This program continues to expand and has become one of the largest VA interventions to assist homeless Veterans. It represents one of VA's most significant efforts to achieve the President's goal of ending homelessness among Veterans. To be eligible for the HUD-VASH program, Veterans must: "Be eligible for VA health care as determined by VA eligibility rules and regulations "Be determined to be homeless based on by the criteria specified by the VA medical center HUD-VASH program with every effort made to identify and serve Veterans deemed to have the highest priority for HUD/VASH services per VA standards and performance measures. Questionnaire for Potential Contractors 1.Does your agency have demonstrated experience providing community based supportive housing placement and case management to VA health care eligible Veterans defined as: "At least one year of experience as an operational Health Care for Homeless Veterans (HCHV) emergency housing provider with an active contract in good standing with the local VA medical center; OR "At least one year of experience as an operational VA Homeless Provider's Grant & Per Diem grantee in good standing with the local VA medical center; OR "Have the capacity to provide HUD-VASH case management services to Hillsborough county, Florida; OR "Other projects or programs in partnership with VA medical centers or the James A. Haley Veterans Hospital deemed relevant by the VA that meet the immediate housing and service needs of homeless Veterans; AND "Recorded attendance at one or more community coalition for the Homeless in the past calendar year 2.Is your agency a direct provider of case management and supportive housing? 3.Is your agency able to subcontract with other agencies meeting the same criteria and capable of performing the work? 4.Please provide documentation that demonstrates your agency's financial capability to fully implement case management services within 30 days of a contract award. 5.Please provide documentation that demonstrates your agency's ability to provide full staffing to include Licensed Clinical Social Worker, Mental Health Counselor, Psychologist, and Housing Specialist based on providing tasks and deliverables associated with "Assertive Community Treatment" (ACT) through existing staff or new hires.
